司库奇尤单抗联合阿维A治疗儿童泛发性脓疱性银屑病合并感染一例

Secukinumab combined with acitretin in treatment of a child with generalized pustular psoriasis complicated with infection

14岁男性患儿,体型肥胖,躯干和四肢红斑、脓疱、脱屑3年,加重1个月,伴高热、寒战。体格检查:体温38.5℃~41.1℃,专科检查:头皮、躯干、四肢可见弥漫性分布鲜红斑、鳞屑,红斑基础上可见粟粒、绿豆大小黄白色脓疱,银屑病三联征阳性,皮损体表面积(BSA)70%。血培养示表皮葡萄球菌生长。诊断:脓疱性银屑病;肥胖症;败血症。经抗感染、阿维A、司库奇尤单抗等治疗达临床治愈。

A 14-year-old obese male patient was admitted due to generalized erythema,pustules,and desquamation for 3 years with exacerbation for a month.The patient also presented with a high fever and shivering.Dermatological findings:diffuse erythema with miliary,mung bean-sized white-yellowish pustules on the surface,squamae,a positive Auspitz’s sign,and skin lesions involving a large body surface area(BSA)of 70%.The blood culture showed growth of Staphylococcus epidermidis.Diagnosis:pustular psoriasis;obesity and sepsis.The patient was clinically cured after clinical treatment with anti-infective agents,acitretin,and secukinumab.
