炎症性肠病患者血清IL-6、sICAM-1及Chemerin的检测及临床价值 被引量:12

摘要 目的:研究炎症性肠病患者血清白介素-6(IL-6)、细胞黏附分子-1(sICAM-1)及脂肪细胞因子Chemerin的检测及临床应用价值。方法:选取炎症性肠病患者63例,其中33例活动期患者作为活动组,30例缓解期患者作为缓解组,另外选择42例健康志愿者作为对照组。比较三组血清IL-6、sICAM-1及Chemerin水平,并研究炎症性肠病患者血清IL-6、sICAM-1及Chemerin水平变化及其与自主神经功能的相关性。结果:炎症性肠病活动组、缓解组以及对照组IL-6、sICAM-1、Chemerin水平依次降低,任意组间比较均有统计学差异(P<0.05);对照组、炎症性肠病缓解组、活动组卧立位血压差依次升高,握力试验血压差及卧立位心率差依次降低,任意组间比较均有统计学差异(P<0.05);三组Valsalva指数经方差分析比较无统计学差异(P>0.05);IL-6、sICAM-1与卧立位血压差成正相关,sICAM-1与卧立位心率差成负相关,均有统计学意义(P<0.05)。结论:炎症性肠病的发病机制与免疫、自主神经功能紊乱、脂质代谢紊乱关系密切,血清IL-6、sICAM-1、Chemerin的检测对炎症性肠病的病情监测有一定的临床意义。 Objective:To study the clinical value of serum interleukin-6(IL-6),cell adhesion molecule-1(sICAM-1)and adipocyte factor Chemerin in patients with inflammatory bowel disease.Methods:63 patients with inflammatory bowel disease were selected as study subjects.33 patients in active phase were divided into active group,30 patients in remission phase were divided into remission group,and 42 healthy volunteers were selected as control group.Serum levels of IL-6,sICAM-1 and Chemerin in three groups were compared,and their correlation with autonomic nerve function were studied.Results:The levels of IL-6,sICAM-1 and Chemerin in active group,remission group and control group decreased in order,and there were statistical differences between any groups(P<0.05).In control group,remission group and active group,the blood pressure difference between the supine and standing positions increased in turn,while the blood pressure difference in grip test and the heart rate difference between the supine and standing positions decreased in turn,and there were statistical differences between any groups(P<0.05).There was no statistical difference between the three groups of Valsalva indexes by variance analysis(P>0.05).The levels of IL-6 and sICAM-1 were positively correlated with the blood pressure difference between the supine and standing positions,and the level of sICAM-1 was negatively correlated with the heart rate difference between the supine and standing positions,with statistical difference(P<0.05).Conclusion:The pathogenesis of inflammatory bowel disease is closely related to immunity,autonomic nervous dysfunction and lipid metabolism disorder.Detection of serum IL-6,sICAM-1 and Chemerin has important clinical significance in monitoring the condition of inflammatory bowel disease.
