无创正压通气治疗老年COPD重症呼吸衰竭患者的疗效观察

Effect of non-invasive positive pressure ventilation on elderly patients with COPD severe respiratory failure

目的:研究无创正压通气治疗老年CPPD重症呼吸衰竭患者的疗效。方法:选取2018年11月-2019年11月收治的老年COPD重症呼吸衰竭患者96例,随机分为对照组和观察组,每组48例。对照组采用常规治疗方法,观察组在常规治疗基础上采用无创正压通气治疗。比较两组的治疗效果,治疗前后的血气指标及肺功能水平。结果:观察组治疗总有效率为87.50%,高于对照组的66.67%,有显著差异(P<0.05)。治疗前两组血气指标相比,无显著差异(P>0.05);治疗后观察组PaO2(80.76±6.34)mmHg、PaCO2(58.10±2.24)mmHg,均优于对照组,有显著差异(P<0.05)。治疗前两组肺功能指标相比,无显著差异(P>0.05);治疗后观察组FEV1(3.24±0.46)L、FVC(3.62±0.25)L、FEV1/FVC(88.78±5.33)%,均优于对照组,有显著差异(P<0.05)。结论:在老年COPD重症呼吸衰竭患者的治疗中,采用无创正压通气治疗方法,能够提高治疗总有效率,改善患者血气指标及肺功能水平,总体疗效比较理想。

Objective:To study the efficacy of non-invasive positive pressure ventilation in the treatment of elderly CPPD patients with severe respiratory failure.Methods:A total of 96 elderly COPD patients with severe respiratory failure admitted from November 2018 to November 2019 were selected and randomly divided into a control group and an observation group,with 48 cases in each group.The control group was treated with conventional treatment,and the observation group was treated with noninvasive positive pressure ventilation on the basis of conventional treatment.Compare the treatment effects of the two groups,blood gas indexes and lung function levels before and after treatment.Results:The total effective rate of treatment in the observation group was 87.50%,which was higher than 66.67%in the control group.There was a significant difference(P<0.05).Before treatment,there was no significant difference in blood gas indexes between the two groups(P>0.05);after treatment,PaO2(80.76±6.34)mmHg and PaCO2(58.10±2.24)mmHg in the observation group were better than those in the control group,with significant differences(P<0.05).Before treatment,there was no significant difference in lung function indexes between the two groups(P>0.05);after treatment,the observation group had FEV1(3.24±0.46)L,FVC(3.62±0.25)L,FEV1/FVC(88.78%±5.33%),all Better than the control group,there is a significant difference(P<0.05).Conclusion:In the treatment of elderly COPD patients with severe respiratory failure,the use of non-invasive positive pressure ventilation therapy can increase the total effective rate of treatment,improve the blood gas indicators and lung function levels of patients,and the overall effect is relatively satisfactory.
