课题达成型品管圈在减重代谢手术患者全程化管理模式构建中的应用

Application of the project-achieving quality control circle in the construction of a whole-course management model for patients undergoing bariatric metabolic surgery

目的基于课题达成型品管圈方法构建减重代谢手术患者全程化管理模式。方法选取2021年1月—2021年8月(改善前组)和2021年12月—2022年3月(改善后组)在南京医科大学附属常州第二人民医院行减重代谢手术的患者各50例为研究对象,成立品管圈小组,选定活动主题为“减重代谢手术患者全程化管理模式的构建”,按照课题达成型品管圈的活动步骤开展活动。通过术后随访及问卷调查,对流程改善前后患者进行追踪,评价活动前后有形成果和无形成果。结果品管圈活动后,患者减重术后3个月多余体重减轻百分比由(49.43±15.79)%提升至(59.67±24.65)%,随访率由62.00%提升至86.00%,患者减重术后知识掌握率由60.00%提升至92.00%,差异均有统计学意义(P<0.05)。构建完成减重代谢手术患者全程化管理模式,修订完善各项流程,全体圈员责任心、品管圈应用手法等各方面能力得到提升。结论通过课题达成型品管圈构建减重代谢手术患者围术期管理模式,规范减重患者围术期护理,加强团队建设,改善减重效果。

Objective To establish a whole-course management model for patients undergoing bariatric metabolic surgery based on the method of project-achieving quality control circle.Methods Fifty patients who underwent bariatric metabolic surgery in the Hospital of Changzhou from January 2021 to August 2021 and from December 2021 to March 2022 were selected as the research subjects.A quality control circle group was established.The activity theme was selected as"construction of a whole-course management model for bariatric metabolic surgery patients",and activities were carried out according to the steps of the project-achieving quality control circle.Patients were tracked before and after the process improvement through postoperative follow-up and questionnaire survey,and tangible and intangible outcomes were evaluated before and after the activity.Results After the quality control circle activity,the percentage of excess weight loss increased from(49.43±15.79)%to(59.67±24.65)%,the follow-up rate increased from 62.00%to 86.00%,and the knowledge mastery rate increased from 60.00%to 92.00%,with statistical significance(P<0.05).The whole-course management model for patients undergoing bariatric metabolic surgery was established and completed,various processes were revised and improved,and the responsibility of all staff members,quality control circle application techniques and other abilities were improved.Conclusions The perioperative management model of patients undergoing bariatric metabolic surgery can be established through the project-achieving quality control circle.The perioperative nursing of patients with weight loss is standardized,the team building is strengthened,and the effect of weight loss is improved.
