腹腔镜右半结肠癌根治性切除术不同入路对比研究

Comparative study of laparoscopic radical resection of right colon cancer by different approaches

目的探究与分析三种不同入路方式下腹腔镜右半结肠癌根治性切除术中的临床效果。方法回顾性分析2017年8月—2022年8月江苏省苏北人民医院收治的右半结肠癌患者167例的临床资料,全部患者均接受了腹腔镜右半结肠癌根治性切除术治疗,该组患者按照手术路径分为中间入路组、尾侧联合中间入路组及头尾联合中间入路组,观察三组患者在一般资料以及围术期指标的差异性。结果头尾联合中间入路组患者在手术时间、术中出血量等情况上优于其他两组(P<0.05);但三者在其他围术期指标上并未呈现出明显的差异性(P>0.05)。结论三种入路在腹腔镜右半结肠癌根治性切除术中均是安全有效的,头尾联合中间入路与其他两种入路方式相比具有更好的手术视野,安全性也较高,围术期综合表现更好。

Objective To explore and analyze the clinical effect of laparoscopic radical resection of right hemicolonic carcinoma with three different approaches.Methods A retrospective analysis was performed on the clinical data of 167 patients with right hemicolon carcinoma admitted to Jiangsu North People's Hospital from August 2017 to August 2022.All patients received radical laparoscopic resection of right hemicolon carcinoma.According to the surgical route,the patients were divided into middle approach group,caudal combined middle approach group and head and tail combined middle approach group.The differences of general data and perioperative indexes among the three groups were observed.Results The operation time and intraoperative blood loss in the head and tail combined middle approach group were better than those in the other two groups(P<0.05).However,there were no significant differences among the three groups in other perioperative indexes(P>0.05).Conclusion The three approaches are safe and effective in laparoscopic radical resection of right hemicolonic carcinoma.Compared with the other two approaches,the head-tail combined middle approach has better surgical field of vision,higher safety and better perioperative performance.
