上海市静安区某社区≥50岁男性高危性行为及影响因素现况调查

Investigation on high-risk sexual behaviors and influencing factors of men≥50 years old in a community of Jing’an District,Shanghai

目的调查上海市静安区某社区≥50岁男性对艾滋病相关知识和相关高危性行为现况,分析其高危影响因素,为该人群以及社区开展艾滋病健康教育措施提供参考和科学依据。方法按照分层抽样的原则,对2019-01-01-2019-10-31居住于上海市静安区某社区≥50岁男性居民进行调查,按11个居委会进行分层分析,共选择550名研究对象。调查内容主要包括人口学资料、艾滋病相关知识、近6个月的性行为等信息,并对≥50岁男性高危性行为的影响因素进行分析。结果回收匿名调查问卷共550份,有效问卷534份,有效率为97.1%,平均年龄(63.5±7.5)岁,总体艾滋病知晓率为86.7%(463/534),近6个月有84.6%(452/534)的研究对象曾与异性或同性发生过性行为,其中高危性行为占17.7%(80/452)。单因素分析结果显示,年龄(χ^(2)=5.480,P=0.019)、婚姻状况(χ^(2)=60.530,P<0.001)、文化程度(χ^(2)=12.685,P=0.002)、居住情况(χ^(2)=186.083,P<0.001)、HIV抗体检查(χ^(2)=136.909,P<0.001)和艾滋病知晓情况(χ^(2)=6.583,P=0.010)均与近6个月发生高危性行为有关,差异有有统计学意义。logistic回归分析结果显示,年龄≥65岁(OR=0.136,95%CI为0.044~0.419,P=0.001)、文化程度为高中或大专(OR=0.208,95%CI为0.052~0.833,P=0.027)、居住情况为伴侣同住(OR=0.018,95%CI为0.006~0.052,P<0.001)、未进行HIV抗体检查(OR=0.047,95%CI为0.018~0.123,P<0.001)是近6个月发生高危性行为的保护因素;婚姻情况为离异/丧偶/未婚(OR=28.870,95%CI为7.260~114.805,P<0.001)是近6个月发生高危性行为的危险因素。结论静安区社区≥50岁男性艾滋病知晓程度较高,但高危性行为的发生率也较高,因此仍需进一步加强健康教育和干预,提高自我保护意识。

Objective To investigate the status of AIDS related knowledge and related high-risk sexual behaviors among men aged≥50 in a community in Jing’an District,Shanghai City,and to analyze the high-risk factors,so as to provide reference and scientific basis for this population and the community to carry out AIDS health education measures.Methods According to the principle of stratified sampling,the male residents aged 50 and above living in Jing’an Temple Community,Shanghai from January 1 to October 31,2019,The analysis was stratified by 11 neighborhood committees,a total of 550.The survey mainly included demographic data,AIDS related knowledge,sexual behavior in the past 6 months and other information,and analyzed the influencing factors of high-risk sexual behavior of men≥50 years old.Results A total of 550 anonymous questionnaires were collected,the result showed effective questionnaires were 534,the effective rate was97.1%,the average age was(63.5±7.5)years old and the overall awareness rate of AIDS was 86.7%(463/534).In the last half year,84.6%(452/534)of the participants had sex,and the high-risk sexual behaviors accounted for 17.7%(80/452).Single factor analysis performed that age(χ^(2)=5.480,P=0.019),marital status(χ^(2)=60.530,P<0.001),degree of education(χ^(2)=12.685,P=0.002),inhabiting information(χ^(2)=186.083,P<0.001),serological test for HIV(χ^(2)=136.909,P<0.001)and the awareness of HIV(χ^(2)=6.583,P=0.010)were significantly associated with the incidence of high-risk sexual behavior.Logistic regression analysis showed that the age was more than 65(OR=-0.136,95%CI0.044—0.419,P=0.001),high school or college education(OR=0.208,95%CI:0.052-0.833,P=0.027),living with partners(OR=0.018,95%CI:0.006-0.052,P<0.001)and no HIV antibody test(OR=0.047,95%CI:0.018-0.123,P<0.001)were the protective factors for high-risk sexual behaviors in recent six months.Divorce/widowhood/unmarried marriage(OR=28.870,95%CI:7.260-114.805,P<0.001)was the risk factor of high-risk sexual behaviors in recent six months.Conclusion Men over 50 yeas old in the Jing’an District community have high awareness of AIDS,but higher incidence of high-risk sexual behaviors,thus health education and intervention should be further strengthened to improve the awareness of self-protection.
