2018-2020年社区老年人肝胆系统疾病现状分析

Analysis on the health examination results of hepatobiliary disease in the community senior citizens,2018-2020

目的了解老年人肝胆系统疾病患病率和人群分布状态,为提升社区卫生工作服务和优化卫生资源配置提供决策依据。方法对2018-2020年昆山市花桥镇20240人次≥65岁老年人的健康体检结果进行χ^(2)或趋势χ^(2)分析。结果脂肪肝、血吸虫肝病和肝囊肿处于老年人肝胆系统患病率前3位。脂肪肝、肝囊肿检出率有所上升,血吸虫肝病检出率逐年降低,差异有统计学意义,均P<0.05。脂肪肝检出率低龄(41.07%)与高龄(22.14%)比较,男性(35.34%)与女性(41.79%)比较,差异有统计学意义,均P<0.05。肝囊肿检出率低龄(15.60%)与高龄(14.07%)比较,男性(18.54%)与女性(12.53%)比较,差异有统计学意义,均P<0.05。血吸虫肝病检出率低龄(21.26%)与高龄(47.83%)比较,男性(27.61%)与女性(21.84%)比较,差异有统计学意义,均P<0.05。结论老年人肝胆系统疾病在不同年份、年龄、性别组的患病率有差异。应持续监测人群患病率变化情况,重点关注检出率呈上升趋势的脂肪肝和肝囊肿,做好关口前移的预防工作。

Objective To investigate in the prevalence and population distribution of hepatobiliary diseases among the elderly,promote community health service and optimize health resources allocation.Methods The physical examination results of 20240 people aged 65 and above in Kunshan Huaqiao Community from 2018 to 2020 were analyzed by chi-square test.Results Fatty liver,schistosoma liver disease and liver cyst were the top three in the prevalence of hepatobiliary system.The prevalence of fatty liver and hepatic cyst increased,while the prevalence of schistosomiasis japonica decreased year by year,all P<0.05.The prevalense of fatty liver of young age was 41.07%,compared with that of old age 22.14%,the male was 35.34%and the female was 41.79%,the difference was statistically significant,all P<0.05.The prevalense of liver cyst of young age(15.60%)was compared with that of old age(14.07%),and that of male(18.54%)was compared with that of female(12.53%),the differences were statistically significant,all P<0.05.The prevalense of schistosomiasis liver disease in young age(21.26%)was compared with old age(47.83%),male(27.61%)compared with female(21.84%),the difference was statistically significant,all P<0.05.Conclusions The prevalence of hepatobiliary diseases in the aged is different in various years,ages and sex groups.The prevalence of fatty liver and liver cyst should be continuously monitored,and the detection rate of fatty liver and liver cyst should be focused on,so as to prevent the advance of the threshold.
