人工流产女性现状调查分析

Investigation and analysis of the current situation of induced abortion women

目的分析女性人工流产现状,降低意外妊娠人工流产数量和重复流产率,提高女性生殖健康水平。方法采用现况调查的方法,对2019-01-31-2019-10-31在郑州大学第三附属医院计划生育门诊自愿接受人工流产的4851例女性进行问卷调查,内容包括年龄、人工流产史、剖宫产史、避孕方式和计划妊娠等,并对调查结果进行统计分析。结果4851例人工流产患者中,<18岁8例(0.16%),18~<30岁2702例(55.70%),30~<40岁1927例(39.73%);无生育史2273例(46.86%);近1年无生育计划者3212例(66.21%);非意愿妊娠4573例(94.27%),其中高危非意愿妊娠1003例(21.93%),意愿妊娠278例(5.73%),其中高危意愿妊娠106例(38.13%),差异有统计学意义,χ2=38.983,P<0.001。非意愿妊娠的主要原因为未避孕(45.79%)和避孕失败(39.67%)。结论人工流产以无生育计划的育龄期女性为主,意愿妊娠患者中高危妊娠占比较非意愿妊娠高,未避孕和避孕失败是导致育龄期女性非意愿妊娠和高危人工流产的主要原因,应继续加大开展流产后关爱优质护理服务,促进高效避孕措施的落实。

Objective Analyzed the current situation of induced abortion,reduced the number of unintended pregnancy abortion and the rate of repeated abortions,and improved female reproductive health.Methods Using the current situation survey method,aquestionnaire survey including age,history of induced abortion,history of cesarean section,contraceptive methods and planned pregnancy was conducted on 4851 women who voluntarily received induced abortions at the Family Planning Clinic of the Third Affiliated Hospital of Zhengzhou University from January 31 to October 31,2019,and conducted statistical analysis of the survey results.Results Among the 4851 induced abortion patients,8(0.16%)were younger than 18 years old,2702 cases(55.70%)were 18-<30 years old,and 1927 cases(39.73%)were 30-<40 years old.2273 cases(46.86%)had no birth history.3212 cases(66.21%)without a childbirth plan in the past year.4573 cases(94.27%)of unwanted pregnancies,including 1003 cases(21.93%)of high-risk unwanted pregnancies and278 cases(5.73%)of intentional pregnancies Among them,106 cases(38.13%)of high-risk willing pregnancy,the difference was statistically significant,χ2=38.983,P<0.001.The main reasons for unintended pregnancy were non-contraception(45.79%)and contraceptive failure(39.67%).Conclusions Induced abortions are mainly women of childbearing age who have no childbearing plans.High-risk pregnancies account for a higher proportion of unintentional pregnancies.Uncontraception and contraceptive failure are the main reasons leading to unintentional and high-risk abortions in women of childbearing age.Carry out post-abortion care and high-quality nursing services to promote the implementation of high-efficiency contraceptive measures.
