我国大陆地区恶性肿瘤患者对社区卫生服务需求质性研究Meta分析

Meta analysis of qualitative research on community health service demand of cancer patients in China's Mainland

目的对我国大陆地区恶性肿瘤患者对社区卫生服务需求的质性研究进行系统评价。方法计算机检索PubMed、Embase、JBI、Web of Science、CINAHL、Academic Search Complete、PsyARTICLES、Cochrane Library、SinoMed、中国知网和万方数据库,搜索关于我国大陆地区恶性肿瘤患者对社区卫生服务需求的质性研究,检索时限均为建库至2020-05-31。纳入标准:研究对象为大陆地区恶性肿瘤患者;感兴趣的现象:恶性肿瘤患者对社区卫生服务的需求;研究设计为质性研究,包括现象学研究和描述性质性研究;研究场所为医院病房、患者家中、临终关怀机构或社区卫生服务中心。排除标准:重复发表的文献;无法提取有效资料的文献;仅量性研究的文献;国外及港澳台地区的研究;非中英文语言发表的文献。采用澳大利亚循证卫生保健中心质性研究质量评价标准对文献进行质量评价后,采用汇集性Meta整合对研究结果进行归纳、整合和诠释。结果共纳入6项研究,提炼出28个完好明确的研究结果,将相似结果归纳形成4个新类别,并综合成2个整合结果:整合结果1为卫生服务需求,整合结果2为心理社会支持需求。结论我国大陆恶性肿瘤患者对社区卫生服务存在需求,应提高社区医疗服务水平,提供社区卫生服务优势下的心理社会支持。

Objective To describe and analyze qualitative studies on demands for community health services in patients with malignant tumors in China's Mainland through meta-synthesis.Methods Databases such as PubMed,Embase,JBI,Web of Science,CINAHL,Academic Search Complete,PsyARTICLES,Cochrane Library,SinoMed,China National Knowledge Infrastructure and Wanfang Data Knowledge Service Platform were retrieved and qualitative studies from the inception to May 2020 on the demands for community health services for patients with malignant tumor in China's Mainland were included.Inclusion criteria:patients with malignant tumors in China's Mainland were as the research objects.The phenomenon of interests:the needs of patients with malignant tumors for community health services;the research design was qualitative research,including phenomenological studies and descriptive studies;the research site was hospital wards,the patient’s home,hospice or community health service center.Exclusion criteria:documents published repeatedly;documents for which valid data cannot be extracted;quantitative research only;researches abroad,Hong Kong,Macao and Taiwan;documents published in languages other than Chinese and English.After evaluating the quality of the literatures using the Australian Evidence-based Health Care Center’s Qualitative Research Quality Evaluation Standards,the collected Meta integration was used to summarize,integrate and interpret the research results.Results A total of 6 studies were included,28 well-defined research results were extracted,similar results were grouped into 4 new categories,and combined into 2 integrated results:integrated result 1 was the need for health services,and integrated result 2 was the need for psychosocial support.Conclusions Patients with malignant tumors in China's Mainland need community health services.It is necessary to improve the level of community medical services and provide psychosocial support with the advantages of primary care.
