摘要
目的妊娠高血压发病率较高,若病情没有及时得到有效控制,则易发展成重度子痫前期,对患者自身及胎儿的生命安全均有严重威胁。本研究分析妊娠高血压进展至重度子痫前期风险因素,旨在为临床诊治提供参考。方法选取2018-02-01-2019-02-01浏阳市妇幼保健院收治的100例妊娠高血压患者为研究对象,选择其中妊娠高血压并重度子痫前期患者60例为病例组,单纯妊娠高血压患者40例为对照组。采用单因素及多因素Logistic回归分析法分析影响妊娠高血压进展至重度子痫前期的独立危险因素。结果单因素分析发现,妊娠高血压并重度子痫前期组收缩压(t=4.839,P<0.001)、尿酸(t=20.823,P<0.001)、血清乳酸脱氢酶(LDH)(t=6.839,P<0.001)和24h尿蛋白定量(t=4.185,P<0.001)水平均与妊娠高血压进展至重度子痫前期有关联。多因素Logistic回归分析显示,收缩压(OR=1.023,95%CI=1.001~1.052,P=0.040)、尿酸(OR=1.006,95%CI=1.003~1.102,P<0.001)和LDH(OR=1.008,95%CI=1.002~1.028,P=0.008)为妊娠高血压进展至重度子痫前期的独立危险因素。结论妊娠高血压进展至重度子痫前期与收缩压、尿酸及LDH水平密切相关。
Objective The incidence of hypertension in pregnancy is relatively high.If the condition is not effectively controlled in time,it will easily develop into severe preeclampsia,which poses a serious threat to the lives of the patient and the fetus.This study analyzed the risk factors for the progression of pregnancy-induced hypertension to severe preeclampsia in order to provide reference for clinical diagnosis and treatment.Methods A total of 100 patients with pregnancy-induced hypertension admitted to Liuyang Maternal and Child Health Hospital from February 1,2018 to February 1,2019 were selected as the research subjects.Among them,60 patients with pregnancy-induced hypertension and severe preeclampsia were selected as the case group,and 40 patients with simple pregnancy-induced hypertension were selected as the control group.Single factor and multivariate logistic regression analysis were used to analyze the independent risk factors affecting the progression of pregnancy-induced hypertension to severe preeclampsia.Results Univariate analysis found that systolic blood pressure(t=4.839,P<0.001),uric acid(t=20.823,P<0.001),serum lactate dehydrogenase(LDH)(t=6.839,P<0.001)and 24-hour urine protein quantification(t=4.185,P<0.001)level were associated with the progression of pregnancy-induced hypertension to severe preeclampsia.Multivariate logistic regression analysis showed that systolic blood pressure(OR=1.023,95%CI=1.001-1.052,P=0.040),uric acid(OR=1.006,95%CI=1.003-1.102,P<0.001)and LDH(OR=1.008,95%CI=1.002-1.028,P=0.008)were independent risk factors for the progression of hypertension in pregnancy to severe preeclampsia.Conclusion The progression of pregnancy-induced hypertension to severe preeclampsia is closely related to systolic blood pressure,uric acid and LDH.
