南通市和平桥街道第一社区卫生服务中心基本公共卫生服务现状分析

Analysis of the status of basic public health services in the first community health service center of Hepingqiao Street in Nantong

目的随着我国卫生改革的深入,社区医疗服务机构作为"第一线"的预防医疗保健服务平台对于构建和谐社会具有重要意义。本研究对南通市和平桥街道第一社区卫生服务中心的基本公共卫生服务现状进行分析,对其存在问题提出政策性建议。方法收集2019-01-01-2019-12-31南通市和平桥街道第一社区卫生服务中心在岗的90名职工和同期收治的999例患者基本信息、就诊原因和就诊需求,并对2016-2019年就诊人数进行描述性统计分析。结果现有在岗职工90人,专业技术人员最多,为71人,其中高级17人,中级25人,初级29人;学历以本科为主,为63人。在岗医生36人,在岗护士18人,医护比2∶1。2016年实行分科,以诊治肿瘤的中医科为特色科室,另设有多种科室。2016-2019年全年门诊挂号人次分别为43810、72279、93446和111384人次,日接待患者分别为120、198、256和305例,呈现逐年升高趋势。999例就诊患者中,≥71岁就诊人数达361例,随着年龄增加,患者就诊需求增大。患病就诊情况以高血压与高血脂并合症最多,为429例(43%)。患者就诊需求中,定期开药老年患者最多,为621例(62%)。结论社区服务中心应优化人才引进政策,吸引高层次医师,增加护士招聘数量,提升其"软实力";推动"主动问诊"的家庭医生模式,增加基层首诊量;加强社区慢性病管理,成为慢性病干预基地。

OBJECTIVE With the deepening of my country’s health reform,community medical service institutions as the"first-line"preventive medical care service platform is of great significance for building a harmonious society.This study analyzes the status of basic public health services in the first community health service center in Hepingqiao Street of Nantong City,and proposes policy recommendations for its existing problems.METHODS Collect the basic information,reasons and needs of 90 staff members and the 999 patients admitted in the First Community Health Service Center of Hepingqiao Street of Nantong City from January 01,2019 to December 31,2019.and give the number of patients in 2016-2019 adescriptive analysis.RESULTS At present,there were 90 on-the-job employees,with the largest number of professional and technical personnel(71),of whom were senior 17,intermediate 25 and junior 29.The education background was mainly undergraduate(63).36 doctors on duty,18 nurses on duty,the ratio of medical care is 2∶1.In 2016,the department was divided into different departments,with the traditional chinese medicine department for diagnosis and treatment of tumors as the characteristic department,and there were many other departments.In 2016-2019,the number of outpatient registrations was 43810,72279,93446 and 111384 times respectively,and the daily reception of patients was about 120,198,256 and 305 cases respectively,showing an increasing trend year by year.Of the 999 patients,361 patients aged 71 or older were in need of medical treatment.Hypertension and hyperlipidemia were the most common diseases,429 cases(43%).Among the patients’demands for medical treatment,621(62%)were elderly patients who prescribed drugs regularly.CONCLUSIONS The community service center should optimize the policy of talent introduction,attract high-level doctors,increase the number of nurses recruited,and improve their"soft power";promote the family doctor mode of"active consultation",increase the number of first visits at the grass-roots level;strengthen the management of chronic diseases in the community,and become a chronic disease intervention base.
