基于数据挖掘探讨中药敷贴治疗膝骨关节炎的用药规律及敷溶介质研究

Study on Medication Rules and Dissolving Media of Traditional Chinese Medicine Application in the Treatment of Knee Osteoarthritis Based on Data Mining

目的探究临床中药敷贴治疗膝骨关节炎(Knee osteoarthritis,KOA)的用药规律及敷溶介质运用情况,为新敷贴方的开发和临床运用提供一定的依据。方法计算机检索数据库中国知网、万方、维普、中国生物医学文献数据库和PubMed,严格按纳入和排除标准,筛选中药敷贴治疗KOA的中药处方,运用Microsoft Excel 2019、SPSS Statistics 25.0、IBM SPSS Modeler 18.0、Cytoscape 3.8.2软件分析并绘制敷溶介质的使用频次及中药的使用频次、中药剂量、功效类别、性味归经、关联规则、核心中药复杂网络、核心中药聚类分析、作用部位-中药聚类分析。结果通过筛选最终纳入201首处方,199味中药,中药使用频次较高的为红花、威灵仙、独活等,药物功效以祛风湿药、活血化瘀药、清热药等为主,药物归经以肝经为主,药性以温为主,药味以辛为主,药物剂量较大。关联规则分析得到药对6条,3味药组35条,4味药组24条,5味药组6条。核心中药复杂网络分析得到27味核心中药。核心中药聚类分析得到中药敷贴治疗KOA潜在4个新组合,作用部位-中药聚类分析得出2个敷贴组合,作用部位为膝关节局部及其局部腧穴。敷溶介质使用频次最多的为蜂蜜。结论中药敷贴治疗KOA时,重在祛风除湿,活血化瘀以治其标实,兼用补肝肾之品以治本虚“,乳香-没药”等配伍最常见。

Objective To explore the medication rules and clinical Chinese medicine application in the treatment of knee osteoarthritis and the application of solution medium,and to provide a basis for the development and clinical application of new external application.Methods Computer search database CNKI,Wanfang,VIP,China Biomedical Literature Database and PubMed.According to the inclusion and exclusion criteria,the prescriptions of external application of traditional Chinese medicine for KOA were screened strictly.Microsoft Excel 2019,SPSS Statistics 25.0,IBM SPSS Modeler 18.0,Cytoscape 3.8.2 software were used to analyze and draw the frequency of use of the dissolution medium,the frequency of use of traditional Chinese medicine,Chinese medicine dose,efficacy categories,taste and meridian tropism,association rules,Chinese medicine complex network,core Chinese medicine cluster analysis and action site-Chinese medicine cluster analysis.Results A total of 201 prescriptions,199 Chinese herbs were finally included through screening.The most frequently used traditional Chinese medicine was Safflower、Chinese clematis root、Double teeth pubescent angelica root,etc.The drug efficacy was given priority to with dispelling wind and dampness,promoting blood circulation and removing blood stasis,clearing heat.The drug meridian was given priority to with liver meridian.The drug property was given priority to with temperature.The drug taste was given priority to with pungent.Larger drug dose.Association rules analysis showed that there were 6 pairs of drugs,35 pairs of 3 herbs,24 pairs of 4 herbs and 6 pairs of 5 herbs.27 Core Traditional Chinese Medicines by Complex Network Analysis.4 potential application combination for KOA Treated by Traditional Chinese Medicine Application Based on Cluster Analysis.Action site-Chinese medicine cluster analysis obtained 2 application combination.Local action site of knee joint and its local acupoints.The most frequently used solvent was honey.Conclusion In the treatment of KOA with traditional Chinese medicine application,the emphasis is on dispelling wind and dampness,promoting blood circulation to remove blood stasis to treat the excess syndrome,and on tonifying liver and kidney to treat root deficiency.The compatibility of"Common monkshood mother-Kusnezoff monkshood motherroot"and"Olibanum-Myrrh"is the most common.
