基于古今文献的运动障碍选穴规律探析

Study on the Point Selection Pattern of Movement Disorders Based on Ancient and Modern Literature

目的通过古今文献共检,分析总结运动障碍的取穴规律,为给运动障碍人群制定安全有效、规范系统的选穴方案提供参考。方法采用文献计量学的方法,对《中华医典》(第五版)及2011-2021年中国期刊全文数据库(CNKI)、维普数据库(VIP)、万方数据资源系统(WF)中运动障碍选穴治疗的相关文献进行检索和整理,分析归纳选穴规律。结果古籍条文纳入443条,共涉及121个腧穴;现代文献纳入249篇,共涉及152个腧穴。使用频率最高的经脉为足少阳胆经和手阳明大肠经;四肢部和头面部取穴较多,躯干部取穴较少;使用频次最高的特定穴为五输穴、下合穴和原穴;曲池、阳陵泉、悬钟、肩髃、手三里、合谷、足三里、环跳、百会、尺泽为古今常用的腧穴。结论古今治疗运动障碍的选穴以运动障碍肢体局部腧穴为主以调形,配合百会穴以调神,体现了"形神共调"的原则。

Objective To analyze and summarize the point selection pattern of movement disorders,and to provide reference for the development of safe and effective,standardized and systematic point selection plan for people with movement disorders through the co-examination of ancient and modern literature.Methods The bibliometric method was used to search and organize the literature related to the selection of acupuncture points for the treatment of movement disorders in Chinese Medical Classics(Version 5.0)and researches published in journals of CNKI,VIP and Wanfang databases from 2011 to 2021 regarding acupoint selection for treating MD were retrieved and sorted out to analyze and summarize the rules of acupoint selection for MD based on bibliometrics.Results A total of 443 items of ancient classics were obtained,involving 121 acupoints.A total of 249 modern researches were included,involving 152 acupoints.The most frequently used meridians are the gall bladder channel of foot-shaoyang and the large intestine meridian of hand-yang-ming.There are more acupoints selected on the limbs and head,and fewer on the trunk.The most frequently used specific acupoints are five-shu points,lower-he-sea points and yuan-primary points.Quchi(LI11),Yanglingquan(GB 34 s),Xuanzhong(GB 39),Jianyu(LI 15),Shousanli(LI 10),Hegu(LI 4),Zusanli(ST 36),Huantiao(GB 30),Baihui(DU 20),Chize(LU 5)are commonly used acupoints in the ancient and modern treatment of MD.Conclusion In ancient and modern times,the selection of acupoints for treating MD is mainly based on the local acupoints on the affected limbs to regulate the body,combining with Baihui(DU 20)to regulate the mind.It reveals the principle of"co-regulation of body and mind".
