放散式体外冲击波穴位治疗联合冲击波压痛点治疗对老年膝骨关节炎患者影响的临床观察

Clinical observation of acupoint therapy of radio extracorporeal shock wave combined with shock wave intervention at tenderness point in the treatment of elderly patients with knee osteoarthritis

目的观察放散式体外冲击波穴位治疗联合冲击波压痛点治疗对老年膝骨关节炎患者的临床疗效。方法将75例老年膝骨关节炎患者随机分为治疗组(39例)和对照组(36例),对照组予放散式体外冲击波压痛点治疗,治疗组在对照组治疗措施的基础上加予冲击波穴位治疗。两组疗程均为8周,比较WOMAC评分、6 min步行距离、骨密度测定值的变化情况。结果①治疗4周、治疗8周后与治疗前组内比较,两组疼痛、僵硬、日常生活难度评分及WOMAC总评分均较治疗前减少(P<0.05);治疗4周、治疗8周后组间比较,治疗组疼痛、僵硬、日常生活难度评分及WOMAC总评分均少于对照组(P<0.05)。②治疗8周后与治疗前组内比较,两组6 min步行距离均增加(P<0.05);组间治疗8周后比较,6 min步行距离差异有统计学意义(P<0.05)。③治疗8周后与治疗前组内比较,两组骨密度测定值差异无统计学意义(P>0.05);组间治疗8周后比较,骨密度测定值差异无统计学意义(P>0.05)。结论放散式体外冲击波穴位治疗联合冲击波压痛点治疗能有效改善膝骨关节炎患者的临床症状,提高运动能力,改善生活质量。

Objective To observe the clinical efficacy of acupoint therapy of radio extracorporeal shock wave combined with shock wave intervention at tenderness point in the treatment of elderly patients with knee osteoarthritis.Methods Seventy-five elderly patients with knee osteoarthritis were randomly divided into the treatment group(39 cases)and control group(36 cases).The control group was treated with radio extracorporeal shock wave at tenderness point,and the treatment group treated with acupoint therapy of radio extracorporeal shock wave based on the treatment for the control group,with a course of 8 weeks.The changes of WOMAC score,6-min walking distance and bone mineral density were compared.Results①Compared with treatment before,the scores of pain,stiffness,difficulty of daily life and the total score of WOMAC in both groups were decreased after treatment for 4 weeks and 8 weeks(P<0.05).After treatment for 4 weeks and 8 weeks,the scores of pain,stiffness,difficulty of daily life and the total score of WOMAC in the treatment group were lower than those in the control group(P<0.05).②Compared with treatment before,the 6-min walking distance in both groups was increased after treatment for 8 weeks(P<0.05).After treatment for 8 weeks,there was statistically significant difference in the 6-min walking distance between the two groups(P<0.05).③In both groups,there was no statistically significant difference in the bone mineral density between treatment before and 8 weeks after treatment(P>0.05).After treatment for 8 weeks,there was no statistically significant difference in the bone mineral density between the two groups(P>0.05).Conclusion Acupoint therapy of radio extracorporeal shock wave combined with shock wave intervention at tenderness point can effectively improve the clinical symptoms,exercise ability and quality of life in patients with knee osteoarthritis.
