朱氏补肾活血方对肾虚血瘀型复发性流产患者妊娠结局及血清炎性因子IL-2、IL-4、IL-10水平观察

Observation of Zhu’s Bushen Huoxue Formula on pregnancy outcome and serum IL-2,IL-4 and IL-10levels of recurrent spontaneous abortion patients with kidney deficiency and blood stasis

目的:观察朱氏补肾活血方对肾虚血瘀型复发性流产(RSA)患者妊娠结局及血清炎性因子白介素(IL)-2、IL-4、IL-10水平。方法:选取RSA患者20例及正常终止妊娠者20例,采用ELISA方法检测两组患者血清中IL-2、IL-4、IL-10水平;选取64例肾虚血瘀型RSA患者,随机分为两组,治疗组与对照组各32例,对照组予口服地屈孕酮常规保胎,治疗组在对照组基础上加中药补肾活血方口服,对比两组患者治疗前后中医证候评分变化及血清水平IL-2、IL-4、IL-10及妊娠结局。结果:①RSA患者外周血中IL-2、IL-4明显高于正常组,IL-10明显低于正常组(P<0.01)。②治疗后,两组中医证候评分、血清IL-2、IL-4水平下降而血清IL-10水平上升,治疗组较对照组结果更加明显,且治疗组活产率高于对照组(P<0.01)。结论:RSA患者血清中IL-2、IL-4明显高于正常组,IL-10明显低于正常组(P<0.01),提示炎性因子IL-2、IL-4、IL-10可能参与RSA的发病相关。朱氏补肾活血方可能通过降低患者外周血中IL-2、IL-4水平、提高IL-10水平,提高RSA患者活产率,改善妊娠结局。

Objective:To observe the effect of Zhu’s Bushen Huoxue Formula on pregnancy outcome and serum interleukin(IL)-2,IL-4 and IL-10 levels in recurrent spontaneous abortion(RSA)patients with kidney deficiency and blood stasis.Methods:Twenty patients with RSA and 20 patients with normal termination of pregnancy were selected,and the serum levels of IL-2,IL-4 and IL-10 in the two groups were detected by ELISA.Sixty-four patients with kidney deficiency and blood stasis RSA were randomly divided into two groups,32 cases in the treatment group and 32 cases in the control group.The control group was given dydrogesterone orally for routine fetal protection,while the treatment group was given traditional Chinese medicine(TCM)Bushen Huoxue Formula orally on the basis of the control group.The changes of TCM syndrome scores,serum levels of IL-2,IL-4,IL-10 and pregnancy outcomes were compared between the two groups before and after treatment.Results:①The levels of IL-2 and IL-4 in peripheral blood of RSA patients were significantly higher than those of normal group,and IL-10 was significantly lower than that of normal group(P<0.01).②After treatment,TCM syndrome score and serum IL-2 and IL-4 levels decreased in both groups,while serum IL-10 level increased.The results in the treatment group were more obvious than those in the control group,and the live birth rate in the treatment group was higher than that in the control group(P<0.01).Conclusion:The serum levels of IL-2,IL-4 and IL-10 in patients with RSA were significantly higher than those in the normal group,while the level of IL-10 were significantly lower than those in the normal group(P<0.01),suggesting that inflammatory factors IL-2,IL-4 and IL-10 may be involved in the pathogenesis of RSA.Zhu's Bushen Huoxue Formula may improve the live birth rate and pregnancy outcome of RSA patients by reducing the levels of IL-2,IL-4 and increasing the level of IL-10 in peripheral blood.
