上颈段脊髓肿瘤手术41例报告

TUMORS OF UPPER CERVICAL MEDULLA SPINALIS IN 41 OPERATIVE CASES

本文报道上颈段肿瘤41例(髓内13例,硬脊膜下27例,硬脊膜外1例),均经手术及病理证实。39例于入院时有不同程度的各种瘫痪。作者采用显微外科技术切除肿瘤,术后24例痊愈,11例好转,4例无改变,3例死亡。作者对手术操作及术后化学治疗中的问题等进行了讨论。

Forty-one cases of tumor of cervical medulla spinalis above Cwere operated upon fin the 12 years from August 1975.Their average age was 36.7 years and duration of disease 26 months.The initial symptom was neck pain in 53.6%,pain in upper extremity 9.7%,paralysis of lower extremity 20%and quadriplegia in 14.6%.During admission,39 of 41 cases had paralysis in various degrees.Definite diagnosis was made through Queckenstedt’s test.All the tumors were excised and some by microsurgical technique.The tumors were located within medulla in 13 cases subdural in 27 and epidural in 1,Two patients(4.8%)died within 3 months,1 died of recurrence half years fter operation,24 recovered motion of the extremities toⅣtoⅤdegree of muscle strength,11 got improvement and 4 was not improved.
