急性产后子宫内翻(附8例报告)

ACUTE POSTPARTUM UTERINE INVERSION A REPORT OF 8 CASES

本文报道上海部分市区及郊区发生的产后急性子宫内翻8例,其中4例于1979年,2例于1977年,1973、1974年各1例.以县为单位进行统计,川沙县发生率为10,644:1,南汇县为20,835:1。8例中死亡4例,死亡率达50%。除一例为自发性子宫内翻,一例为粘膜下肌瘤导致子宫内翻外,其余6例均与第三产程处理不当有关。强压宫底、强力牵拉脐带是造成内翻的主要原因.另对子宫内翻的诱因、刺激因素、症状、预防及处理结合文献加以讨论.

This article presented 8 cases of acute postpartum uterine inversion seen mainly in rural district of Shanghai during 1973-1979.There were 4 deaths,with a mortality as high as 50%.The causes of inversion were:ill-management during the third stage labor(6 cases),spontaneous inversion(1 case)and prolapse of large submucous myoma(1 case).Combined forciful tractions on umbilical cord and pushing of fundus of uterus to hasten the delivery of placenta were the mechanism of inversion.The prevention and early diagnosis were briefly discussed.
