完全性左束支传导阻滞——50例心向量图与心电图分析

COMPLETE LEFT BUNDLE BRANCH BLOCK, ELECTROCARDIOGRAPHIC AND VECTORCARDIOGRAPHIC ANALYSIS OF 50 CASES

完全性左束支传导阻滞的心向量图诊断已屡见报道。近年来,关于完全性左束支传导阻滞的病因[1,2]、预后[3]均有不同的看法。

The electrocardiogram and veotorcardiogram of fifty cases with complete LBBB were analyzed,and correlated with the clinical status.According to the direction of maximal QRS vector in frontal plane VCG and mean electrical axis in ECG,patients could be divided into two groups.Group A(25 cases)revealed complete LBBB with normal electrical axis and group B(25 cases)complete LBBB with left axis deviation(≥-30°).Complications of enlargement of heart,heart failure,coronary artery disease and A-V block were found more frequent in group B patients than in group A.Although there were no definite correlations between the prognosis and the pattern of EGG and VCG changes among these patients,it could be suggested that myooardial lesion,including that of conduction system,may be more diffuse and severe in those patients with complete LBBB accompanied by left axis deviation illustrated in electrocardiogram.
