胰岛素泵基础率不同分段法对2型糖尿病患者血糖控制的疗效观察 被引量:1

摘要 目的胰岛素泵基础率不同分段法对2型糖尿病患者血糖控制的疗效。方法随机将150例2型糖尿病患者分为三组,每组50例。A,B和C组的胰岛素泵基础率分别为3段、5段与24段,比较治疗前后各组血糖变化及治疗情况(包括达标时间、胰岛素用量以及低血糖发生率)。结果治疗前各组BMI、FPG、2hPG、HB A1C、空腹、早餐后、午餐后、晚餐后、凌晨3点血糖均无明显差异(P>0.05)。治疗后各组患者血糖均有明显改善,各时间点血糖比较无明显差异。经重复方差分析三组患者血糖控制达标时间、低血糖发生率、胰岛素用量均存在明显差异(P<0.05),C组患者血糖控制达标时间、低血糖发生率、胰岛素用量均明显少于A、B两组患者(P<0.05),具有统计学意义。结论24段法胰岛素泵疗法治疗Ⅱ型糖尿病血糖控制达标时间短、低血糖发生率低、胰岛素用量少,效果明显优于3段和5段法,但其设置方法繁琐,可结合具体情况采取相应的方案进行操作。 Objective To study the effect of different insulin pump basal volume method on blood glucose control in type 2 diabetic patients.Methods 150 patients with type 2 diabetes were randomly divided into three groups and 50 cases were in each group.Patients with insulin pump basal volume was 3,5 and 24 segments in group A,B and C,respectively.Blood glucose changes and treatment(including the standard time,insulin dosage and the incidence of hypoglycemia)before and after treatment were compared between each group.Results There was no significant difference in BMI,FPG,2 hPG,HB A1 C and blood sugar at fasting,after breakfast,lunch,dinner and at three o’clock in the morning in all groups before treatment(P>0.05).The blood glucose of patients in each group was significantly improved after treatment than those before treatment(P<0.05).There was no significant difference in blood glucose of the patients in each time point after treatment(P>0.05).After repeated analysis of variance,there were significant differences in blood glucose control time,incidence of hypoglycemia and insulin dosage between three groups(P<0.05).The blood glucose control time,the incidence of hypoglycemia and insulin dosage in group C were significantly less than those in group A and B(P<0.05).Conclusion The 24-stage of insulin therapy for type 2 diabetes has a short blood sugar control standard time,low hypoglycemia rate and less dosage of insulin.The effect is significantly better than 3 and 5 stages,but its setting method is cumbersome and can be adopted according to the specific situation.
