阴道高频彩色多普勒超声诊断鉴别子宫内膜息肉样病变良恶性的价值分析

Value Analysis of Transvaginal High Frequency Color Doppler Ultrasound in Differentiating Benign and Malignant Endometrial Polypoid Lesions

目的探讨阴道高频彩色多普勒超声对子宫内膜息肉样病变良恶性的鉴别诊断价值。方法抽取87例初诊为子宫内膜息肉的患者,术前均行阴道高频彩色多普勒超声诊断,以手术病理结果作为金标准,评价其对子宫内膜息肉样病变良恶性的鉴别价值。结果手术病理诊断恶性15例,良性72例。阴道高频超声诊断准确率为96.55%(84/87),特异度为97.22%(70/72),敏感度为93.33%(14/15),与手术病理诊断一致性较好(Kappa值=0.701、0.709、0.697)。良性病变患者与恶性病变患者相比内膜厚度、RI有显著差异(P<0.05),PSV差异不显著(P>0.05)。结论阴道高频彩色多普勒超声对子宫内膜息肉样病变良恶性鉴别价值较高,诊断准确率、敏感度等与手术病理诊断一致性较好,值得应用。

Objective To investigate the value of high frequency color Doppler ultrasound in the differential diagnosis of benign and malignant endometrial polypoid lesions.Methods 87 cases of endometrial polyps were diagnosed with high-frequency color Doppler ultrasound before operation.The pathological results were used as the gold standard to evaluate the differential value of benign and malignant endometrial polypoid lesions.Results 15 cases were malignant and 72 cases were benign.The accuracy,specificity and sensitivity of transvaginal ultrasonography were 96.55%(84/87),97.22%(70/72)and 93.33%(14/15),respectively,which were consistent with pathological diagnosis(Kappa=0.701,0.709,0.697).Compared with patients with malignant lesions,patients with benign lesions had significant differences in intimal thickness and RI(P<0.05),while the difference in PSV was not significant(P>0.05).Conclusion Vaginal high frequency color Doppler ultrasound has high value in differentiating benign and malignant endometrial polypoid lesions.The diagnostic accuracy and sensitivity are consistent with surgical pathology,and it is worth applying.
