COPD急性发作期中医护理优化方案临床疗效研究

Study on Clinical Efficacy of TCM Nursing Optimization Scheme in AECOPD

目的:探讨子午流注择时护理治疗COPD急性发作期患者的临床疗效。方法:将72例病人随机分为观察组、对照组各36例。对照组给予西医常规支持、对症治疗,包括抗感染、解痉平喘及化痰药物、雾化吸入、氧疗、营养支持等治疗。观察组在对照组基础上增加实施基于“三因制宜”治则和“子午流注”原理的中医护理优化方案,包括穴位排痰、改良穴位贴敷、穴位按摩、雾化吸入、坐式呼吸操等5项适宜技术,均治疗10d。比较两组治疗前后的临床疗效、肺功能的变化。结果:干预10d后,观察组的总体疗效、肺功能改善均显著优于对照组(P<0.05)。结论:基于“三因制宜”治则和“子午流注”原理对COPD急性发作期中医护理方案的优化,提高针对性和可操作性,能够有效改善COPD急性发作期病人的临床症状、肺功能水平,促进康复。

Objective:This study aims to investigate the clinical efficacy of the Midnight-Midday Ebb Flow timing nursing in treating patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:A total of 72 patients with AECOPD were randomly assigned to the observation group(n=36)and control group(n=36).The control group received conventional western medicine treatment,including anti-infection,antispasmodic,antiasthmatic and expectorant drugs,aerosol inhalation,oxygen therapy,and nutritional support.The observation group received the optimized traditional Chinese medicine(TCM)nursing program based on the“Three Factors Adapting Measures”and the“Midnight-Midday Ebb Flow”principle,which included five TCM techniques,namely acupoint expectoration,improved acupoint application,acupoint massage,atomization inhalation,and sitting breathing exercises.Both groups were treated for 10 days,and the clinical efficacy and changes in lung function were compared before and after treatment.Results:After 10 days of intervention,the overall efficacy and pulmonary function improvement in the observation group were significantly better than those in the control group(P<0.05).Conclusions:The optimization of the TCM nursing program for AECOPD based on the principle of“Three Factors Adapting Measures”and“Midnight-Midday Ebb Flow”has stronger pertinence and operability,effectively improving the clinical symptoms and lung function level.This study highlights the potential benefits of incorporating TCM nursing techniques in the treatment of AECOPD.
