利胆护胆汤联合牛磺熊去氧胆酸在预防内镜微创保胆取石术后胆囊结石复发的临床疗效观察

Comparison of the clinical effect of Lidan Hudan Decoction and Taurine deoxycholic acid in preventing recurrence of cholecystolithiasis after endoscopic minimally invasive cholecystectomy

目的:探究利胆护胆汤联合牛磺熊去氧胆酸在预防内镜微创保胆取石术后胆囊结石复发的临床疗效。方法:选择2019年1月~2020年6月保定市第二医院收治的胆结石患者120例,按照治疗方式不同分为对照组和观察组,每组60例。两组患者行内镜微创保胆取石术后,对照组患者服用牛磺熊去氧胆酸,观察组患者在对照组基础上加用利胆护胆汤,观察两组患者治疗后术后总有效率、胆囊结石复发率、胆囊壁厚度及胆囊收缩率及不良反应发生率。结果:经治疗后,观察组患者总有效率显著高于对照组患者(P<0.05);观察组患者胆囊结石复发率显著低于对照组患者(P<0.05);两组患者的胆囊壁厚度显著降低,且观察组患者降低显著高于对照组,胆囊收缩率则显著升高,且观察组患者比对照组患者升高明显(P<0.05),两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:利胆护胆汤能够显著预防内镜微创保胆取石术后胆囊结石的复发,改善胆囊壁厚度和胆囊收缩功能,安全有效,值得临床推广使用。

Objective To explore the clinical efficacy of Lidan Hudan Decoction combined with tauro-ursodeoxycholic acid in preventing recurrence of gallbladder stones after endoscopic minimally invasive biliary stone extraction.Methods From January 2019 to June 2020,120 patients with gallstones in our hospital were divided into two groups:Lidan Hudan decoction and taurodeoxycholic acid,60 cases in each group.Two groups of patients were treated with endoscopic minimally invasive gallbladderpreserving lithotomy,the control group took ttaurodeoxycholic acid,and the observation group added Lidan Hudan Decoctionn on the basis of the control group.The total postoperative efficiency,the recurrence rate of gallstone,the thickness of gallbladder wall,the rate of gallbladder contraction and the incidence of adverse reactions were observed after treatment.Results After treatment,the total effective rate of the patients in the observation group was significantly higher than that of patients in the control group(P<0.05);The recurrence rate of cholecystolithiasis in the observation group was significantly lower than that in the control group(P<0.05);The gallbladder wall thickness was significantly reduced in both groups,and the reduction was significantly higher in the observation group than in the control group,while the gallbladder contraction rate was significantly higher in the observation group than in the control group(P<0.05);There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Lidan Hudan decoction can significantly prevent the recurrence of cholecystolithiasis after endoscopic minimally invasive cholecystectomy,improve the thickness of gallbladder wall and gallbladder contraction function,which is safe and effective,and is worthy of clinical application.
