菖蒲郁金汤辅助治疗对重症病毒性脑炎的中医证候、神经功能及炎症因子的影响

Effect of Changpu Yujin Decoction on Traditional Chinese Medicine Syndrome, Neurological Function and Inflammatory Factors for Severe Viral Encephalitis

目的:探讨菖蒲郁金汤辅助治疗重症病毒性脑炎的疗效及对中医证候、神经功能及炎症因子的影响。方法:选取2015年12月~2020年5月无锡市太湖医院收治的80例重症病毒性脑炎患儿,将其随机分为对照组(n=40)与观察组(n=40)。对照组采取常规对症治疗,观察组在对照组基础上联合菖蒲郁金汤辅助治疗。评估两组临床疗效、中医证候及不良反应;比较两组治疗前后神经功能指标[人脑髓鞘碱性蛋白(MBP)、神经生长因子(NGF)、神经元特异性烯醇化酶(NSE)、中枢神经特异性蛋白(S100-β蛋白)]及炎性因子[肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)]水平。结果:观察组治疗总有效率显著高于对照组(P<0.05);观察组治疗后中医证候改善程度显著高于对照组(P<0.05);观察组患儿MBP、NGF、NSE、S100-β蛋白、IL-6和TNF-α等指标改善程度显著高于对照组(P<0.05);两组治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论:菖蒲郁金汤辅助治疗重症病毒性脑炎疗效和安全性均较高,可有效改善患儿中医证候,调节神经相关蛋白表达及减轻炎性反应。

Objective:To investigate the effect of using Changpu Yujin Decoction in treating severe viral encephalitis and its influence on TCM syndrome,neurological function and inflammatory factors.Methods:A total of 80 patients with severe viral encephalitis in our hospital from December 2015 to may 2020 were selected and randomly divided into a western medicine group(n=40)and a traditional Chinese and western medicine group(n=40).The western medicine group was treated with conventional symptomatic treatment,and the western medicine group was treated with Changpu Yujin Decoction.The clinical efficacy,TCM syndromes and adverse reactions of the two groups were evaluated.The levels of nerve function indexes(MBP,NGF,NSE,S100-βprotein)and inflammatory factors(TNF-α,IL-6)were compared between the two groups before and after treatment.Results:The total effective rate of the western medicine group was significantly higher than that of the western medicine group(P<0.05).After treatment,the improvement of TCM syndrome in the TCM group was significantly higher than that in the WM group(P<0.05).The improvement degree of MBP,NGF,NSE,S100-βprotein,IL-6 and TNF-αin the western medicine group was significantly higher than that in the western medicine group(P<0.05).There was no adverse reaction in the two groups.Conclusion:Changpu Yujin Decoction has high efficacy and safety in the adjuvant treatment of severe viral encephalitis,which can effectively improve the TCM syndrome,regulate the expression of nerve related proteins and inflammatory reaction.
