清浊益肾汤对痰湿瘀热型糖尿病肾病患者肾功能、糖脂代谢、炎症及氧化应激因子的影响

Effect of Qingzhuo Yishen Decoction on Renal Function,Glucose and Lipid Metabolism,Inflammation and Oxidative Stress Factorsfor Patients with Diabetic Nephropathy of Phlegm Dampness Stasis Heat

目的:探讨清浊益肾汤对痰湿瘀热型糖尿病肾病(DN)患者肾功能、糖脂代谢、炎症及氧化应激因子的影响。方法:选取2018年1月~2020年1月沧州中西医结合医院内分泌科收治的140例DN患者,根据治疗方法不同分为两组,对照组(n=70)接受常规治疗,观察组(n=70)在对照组基础上接受清浊益肾汤治疗。对比两组临床疗效和治疗后中医症状积分、肾功能、糖脂代谢、炎症、氧化应激指标及不良反应。结果:观察组临床治疗总有效率明显高于对照组(P<0.05);治疗后观察组中医症状积分、血肌酐(SCr)、血尿素氮(BUN)、胱抑素-C(Cys-C)、尿白蛋白排泄率(UAER)、空腹血糖(FBG)、2hFBG、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、超氧化物歧化酶(SOD)水平明显低于对照组,高密度脂蛋白胆固醇(HDL-C)、谷胱甘肽过氧化物酶(GSH-PX)、丙二醛(MDA)水平明显高于对照组(P<0.05);两组患者均无严重不良反应出现。结论:清浊益肾汤治疗痰湿瘀热型DN疗效确切,可显著改善患者临床症状、肾功能、糖脂代谢,抑制炎症反应和氧化应激,且安全性高,值得推广。

Objective:To explore the effect of Qingzhuo Yishen Decoction on renal function,glucose and lipid metabolism,inflammation and oxidative stress factors for patients with phlegm-dampness stasis-heat type diabetic nephropathy(DN).Methods:A total of 140 DN patients admitted to our hospital(January 2018 to January 2020)were selected and divided into two groups according to the random number table method.The controlled group(n=70)received conventional treatment,and the observation group(n=70)on the basis of the controlled group,they received treatment with Qingzhuo Yishen Decoction.The clinical efficacy and TCM symptom scores,renal function,glucose and lipid metabolism,inflammation,oxidative stress indicators and adverse reactions after treatment were compared between the two groups.Results:The total effective rate of clinical treatment in the observation group was significantly higher than that in the controlled group(P<0.05).After treatment,the observation group’s TCM symptom score,blood creatinine(SCr),blood urea nitrogen(BUN),cystatin-C(Cys-C),urine albumin excretion rate(UAER),fasting blood glucose(FBG),2 hFBG,hemoglobin A1 c(HbA1 c),total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),and superoxide dismutase(SOD)levels were significantly lower than those of the controlled group,and high-density lipoprotein cholesterol(HDL-C),glutathione peroxidase(GSH-PX),and malondialdehyde(MDA)levels were significantly higher than those of the controlled group(P<0.05).There were no serious adverse reactions in both groups.Conclusion:Qingzhuo Yishen Decoction is effective in treating DN with phlegm-dampness stasis-heat type.It can significantly improve clinical symptoms,renal function,glucose and lipid metabolism,inhibit inflammation and oxidative stress,and is highly safe.It is worthy of promotion.
