二陈汤加味联合二甲双胍片对超重肥胖2型糖尿病(脾虚痰湿型)患者的影响

Effect ofModified Erchen Decoction Combined with Metformin Tablet for Patients with Overweight and Obese Type 2 Diabetes(Spleen Deficiency and Phlegm-dampness Type)

目的:观察二陈汤加味联合二甲双胍片对超重肥胖2型糖尿病(脾虚痰湿型)患者的影响。方法:选择2019年3月~2021年2月在我院治疗的超重肥胖2型糖尿病患者110例,随机分为观察组和对照组,各55例,对照组进行二甲双胍片疗法,观察组进行二陈汤加味联合二甲双胍片联合疗法。结果:服药前,两组血糖指标比较无统计学差异(P>0.05);服药后,两组血糖指标均较治疗前有所下降,观察组的FPG、P2hBG、HbA1c显著低于对照组,有统计学意义(P<0.05);服药前,两组BMI以及C-肽的对比无显著性差异(P>0.05),两组具有可比性;服药后,观察组BMI降低程度显著高于对照组(P<0.05),观察组的CP0以及CP2升高程度显著高于对照组(P<0.05);服药前,两组胰岛素指标及胰岛素抵抗指数在统计学上无差异(P>0.05),服药后,两组胰岛素指标及及胰岛素抵抗指数均较治疗前下降,观察组的FINS、P2hINS以及HOMA-IR显著低于对照组(P<0.05);服药前,两组血脂指数在统计学上无差异(P>0.05),治疗后,两组TG以及LDL-C均较治疗前下降,HDL-C较治疗前升高,观察组的TG、LDL-C以及HDL-C改善程度显著高于对照组(P<0.05)。结论:对超重肥胖2型糖尿病(脾虚痰湿型)患者进行二陈汤加味联合二甲双胍片联合疗法,可显著控制血糖水平,降低机体胰岛素抵抗,控制体重,临床效果显著,值得临床推广使用。

Objective:The effect of Modified Erchen Decoction combined with Metformin Tablet for patients with overweight and obese type 2 diabetes mellitus(spleen deficiency phlegm-dampness type).Methods:10 overweight and obese type 2 diabetes patients with diabetes mellitus(spleen deficiency and phlegm-dampness type)admitted to our hospital from March 2019 to February 2021.There was 55 cases in each group.The controlled group received Metformin therapy,while the observation group received Modified Erchen Decoction combined with Metformin Tablet combined therapy.Blood glucose indexes(FPG,P2 HBG,HbA1 c),body mass index(BMI),C-peptide,insulin index(FINS,P2 HINS),insulin resistance index(HOMA-IR),plasma lipid metabolism C-peptide[fasting C-peptide(CP0),postpranadial 2 HC peptide(CP2)]were compared between 2 groups before and after treatment.Blood lipid metabolism(TG,LDL-C,and HDL-C)levels.Results:Before medication,there was no significant difference in blood glucose indexes between the two groups(P>0.05).After medication,blood glucose indexes in both groups decreased compared with before treatment,and FPG,P2 HBg and HbA1 c in the observation group were significantly lower than those in the controlled group,with no statistical difference(P<0.05).Before medication,there was no significant difference in BMI and C-peptide between the two groups(P>0.05),indicating that the two groups were comparable.After medication,the decrease degree of BMI in the observation group was significantly higher than that in the controlled group(P<0.05),and the increase degree of CP0 and CP2 in the observation group was significantly higher than that in the controlled group(P<0.05).Before medication,there were no significant differences in insulin index and insulin resistance index between the two groups(P>0.05).After medication,insulin index and insulin resistance index were decreased in the two groups,while FINS,P2 HINS and HOMA-IR in the observation group were significantly lower than those in the controlled group(P<0.05).Before treatment,there was no statistically significant difference in lipid index between the two groups(P>0.05).After treatment,TG and LDL-C in the two groups were decreased and HDL-C was increased compared with before treatment.The improvement degree of TG,LDL-C and HDL-C in the observation group was significantly higher than that in the controlled group(P<0.05).Conclusion:For patients with overweight and obese patients with diabetes mellitus(spleen deficiency and phlegm-dampness type),Modified Erchen Decoction combined with Metformin Tablet combined therapy can significantly control the blood glucose level,reduce the body’s insulin resistance and control body weight,with significant clinical effect and worthy of clinical promotion.
