太溪穴平刺联合中极穴齐刺治疗前列腺增生症患者经尿道前列腺电切术后并发尿潴留的疗效观察

Curative Observation of Applying Taixi Point Horizontal Insertion of Needle Combined with Zhongji Point Triple Puncture in the Treatment of Urinary Retention after Transurethral Resection of Prostate in Patients with Benign Prostatic Hyperplasia

目的:探究常太溪穴平刺联合中极穴齐刺治疗前列腺增生症(BPH)患者经尿道前列腺电切(TURP)术后并发尿潴留肾虚血瘀证的疗效观察。方法:选取2020年5月~2021年5月于本院行TURP术后并发尿潴留的肾虚血瘀证BPH患者84例,按随机数字表法分为观察组42例和对照组42例,对照组采取常规针刺进行治疗,观察组在对照组基础上采用太溪穴平刺联合中极穴齐刺进行治疗,观察两组临床疗效,比较治疗前后患者中医症候积分、国际前列腺症状(IPSS)评分、前列腺体积、最大尿流率(Q_(max))、尿量残余量变化。结果:观察组临床总有效率为92.86%,明显高于对照组的78.57%,具有统计学意义(P<0.05);治疗后观察组夜尿频繁、排尿困难、尿线情况、腰膝酸软及少腹胀痛等中医症候积分评分均明显低于对照组(P<0.05);治疗后,观察组IPSS评分、前列腺体积明显低于对照组(P<0.05);治疗后,观察组Q明显高于对照组,残余尿量明显低于对照组(P<0.05)。结论:太溪穴平刺联合中极穴齐刺治疗BPH患者TURP术后并发尿潴留肾虚血瘀证疗效显著,可促进患者术后前列腺功能恢复,缓解尿潴留症状,具有较高的临床推广价值。

Objective:To explore the curative effect of Taixi point horizontal insertion of needle combined with Zhongji point triple puncture for patients with benign prostatic hyperplasia(BPH)after urethra resection of prostate(TURP)complicated with urinary retention and kidney deficiency and blood stasis syndrome.Methods:84 patients with BPH deficiency and blood stasis syndrome who underwent TURP after May 2020 were selected and divided into an observation group(42 cases)and a controlled group(42 cases)according to random number table.The controlled group was treated with conventional acupuncture.The observation group was treated with the combination of the acupuncture at Taixi point and Zhongji point,on the basis of the controlled group,and the clinical efficacy of the two groups was observed.The controlled group was treated with routine acupuncture.The observation group was treated with the combination of the acupuncture at Taixi point and Zhongji point.The clinical efficacy of the two groups was observed.The changes of TCM symptom score,international prostate symptom(IPSS)score,prostate volume,maximum urinary flow rate(Qmax)and residual urine volume were compared before and after treatment.Results:The total clinical effective rate in the observation group was 92.86%,which was significantly higher than 78.57%in the controlled group(P<0.05).After treatment,the scores of TCM symptoms such as frequent nocturia,dysuria,urinary line,sore waist and knees and less abdominal pain in the observation group were significantly lower than those in the controlled group(P<0.05).After treatment,IPSS score and prostate volume in the observation group were significantly lower than those in the controlled group(P<0.05).After treatment,Qmax in the observation group was significantly higher than that in the controlled group,and the residual urine volume was significantly lower than that in the controlled group(P<0.05).Conclusion:The acupuncture at Taixi point combined with Zhongji point needling therapy is effective in treating BPH patients with TURP after operation.It can promote the recovery of prostate function and relieve the symptoms of urinary retention,and has a high clinical value.
