穴位埋线联合调任通督针刺法对脾肾阳虚型早发性卵巢功能不全患者卵巢功能的影响分析

Effect Analysis of Acupoint Catgut Embedding Combined with Tiaoren Tongdu Acupuncture on Ovarian Function for Patients with Spleen-kidney Yang-deficiency Type Premature Ovarian Insufficiency

目的:研究穴位埋线联合调任通督针刺法对脾肾阳虚型早发性卵巢功能不全(POI)卵巢功能的影响。方法:将145例脾肾阳虚型POI患者随机分为对照组72例与观察组73例,对照组采用调任通督针刺法治疗,隔日1次,1个月为1疗程;观察组在对照组的治疗基础上加用穴位埋线治疗,每两周1次,4次为1疗程。2组均治疗12周后观察疗效,比较两组治疗前后的临床效果、中医临床症状、基础血清性激素变化情况以及卵巢功能情况。结果:治疗后,两组临床有效率分别为97.26%和88.89%,差异提示有统计学意义(P<0.05);两组中医临床症状积分均明显降低(P<0.05),且观察组相较对照组而言降低更明显(P<0.05);治疗后,观察组的促卵泡成熟激素(FSH)、促黄体生成素(LH)水平比对照组的水平低(P<0.05),雌二醇(E2)水平比对照组水平高(P<0.05);治疗后,两组Kupperman自评量表积分均低于治疗前(P<0.05),且观察组低于对照组(P<0.05);治疗后两组卵巢功能明显改善,抗缪勒管激素(AMH)水平均高于治疗前(P<0.05),且观察组高于对照组(P<0.05)。结论:“调任通督针刺法”联合穴位埋线对于脾肾阳虚型POI患者具有很好的疗效,在改善患者的临床症状、调节机体激素水平以及卵巢功能水平上优于单纯针刺治疗,有利于提高育龄患者生育率,提高生活质量。

Objective:To study the effect of acupoint catgut embedding combined with Tiaoren Tongdu acupuncture on the ovarian function of premature ovarian insufficiency(POI)of spleen-kidney Yang-deficiency.Methods:145 patients with POI of spleen-kidney Yang-deficiency were randomly divided into 72 cases in a controlled group and 73 cases in an observation group.The controlled group was treated with Tiaoren Tongdu acupuncture(once every other day and 1 month as a course of treatment),and the observation group was additionally given acupoint catgut embedding on the basis of the controlled group(once every two weeks and 4 times as a course of treatment).The efficacy after 12 weeks of treatment was observed in both groups,and the clinical effect,TCM clinical symptoms,changes of basic serum sex hormones and ovarian function were compared before and after treatment in the two groups.Results:After treatment,the clinical effective rates of the two groups were 97.26%and 88.89%respectively(P<0.05).The scores of clinical symptoms in the two groups were significantly reduced(P<0.05),and the decreases were more significant in the observation group compared to the controlled group(P<0.05).After treatment,the levels of follicle-stimulating hormone(FSH)and luteinizing hormone(LH)in the observation group were lower than those in the controlled group(P<0.05)while the level of estradiol(E)was higher than that in the controlled group(P<0.05).After treatment,the Kupperman self-rating scale scores of the two groups were lower than those before treatment(P<0.05),and the score of the observation group was lower than that of the controlled group(P<0.05).The ovarian function of the two groups was significantly improved after treatment,and the level of anti-Mullerian hormone(AMH)was higher than that before treatment(P<0.05),and the level of the observation group was higher than that of the controlled group(P<0.05).Conclusion:Tiaoren Tongdu acupuncture combined with acupoint catgut embedding has a good efficacy for patients with POI of spleen-kidney Yang-deficiency.It is better than simple acupuncture in improving the clinical symptoms and regulating the levels of body hormones and ovarian function,and it is beneficial to increasing the fertility rate of patients of childbearing age and improving the quality of life.
