白驳汤联合窄谱中波紫外线治疗白癜风的临床疗效及对免疫功能的影响

Clinical Efficacy of Applying Baipi Decoction Combined with Narrow-band Ultraviolet in the Treatment of Vitiligo and Its Influence on Immune Function

目的:探讨白驳汤联合窄谱中波紫外线(NB-UVB)治疗白癜风的临床疗效及对免疫功能的影响。方法:选取2020年1月~2020年12月上海中医药大学附属曙光医院宝山分院收治的80例白癜风患者,随机数字表法分为两组,对照组(n=40)接受NB-UVB治疗,观察组(n=40)接受白驳汤联合NB-UVB治疗。对比两组患者临床疗效和治疗后中医症状积分、生活质量、皮损面积、外周血T淋巴细胞、血清免疫球蛋白(Ig)水平及不良反应发生情况。结果:观察组临床治疗总有效率明显高于对照组(P<0.05);治疗后观察组中医症状积分、皮肤病生活质量指数(DLQI)评分和外周血CD8^(+)百分比明显低于对照组,皮损面积明显小于对照组,外周血CD4^(+)、CD4^(+)/CD8^(+)、调节性T细胞(Treg)百分比和血清IgA、IgM、IgG水平明显高于对照组(P<0.05)。两组患者不良反应发生率对比无差异(P>0.05)。结论:白驳汤联合NB-UVB治疗白癜风疗效确切,可显著改善皮损,提升生活质量,调节免疫功能,安全性高,值得推广。

Objective:To explore the clinical efficacy of applying Baipi Decoction combined with narrow-band ultraviolet(NB-UVB)in the treatment of vitiligo and its influence on immune function.Methods:Eighty patients with vitiligo admitted to our hospital(January 2020 to December 2020)were selected and divided into two groups according to the random number table method.The controlled group(n=40)received NB-UVB treatment,and the observation group(n=40)received Baibo Decoction combined with NB-UVB treatment.The clinical efficacy and TCM symptom scores,quality of life,skin lesion area,peripheral blood T lymphocytes,serum immunoglobulin(Ig)levels,and adverse reactions after treatment were compared between the two groups.Results:The total effective rate of clinical treatment in the observation group was significantly higher than that in the controlled group(P<0.05).After treatment,the observation group's TCM symptom score,Dermatology Life Quality Index(DLQI)score and peripheral blood CD_(8)^(+)percentage were significantly lower than those of the controlled group,and the area of skin lesions was significantly smaller than that of the controlled group.Peripheral blood CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),T regulatory cell(Treg)and serum IgA,IgM and IgG levels were significantly higher than those of the controlled group(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Using Baibo Decoction combined with NB-UVB has a definite effect in the treatment of vitiligo,which can significantly improve skin lesions,improve the quality of life,regulate immune function,and has high safety,it is worth promoting.
