健脾消积汤联合西药治疗原发性肝癌癌因性疲劳的疗效及对患者生存期和生活质量的影响

Efficacy of Using Jianpi Xiaoji Decoction Combined with Western Medicine in the Treatment of Primary Hepatocellular Carcinoma Induced Fatigue and Its Inflence on Patients’ Survival Time and Quality of Life

目的:探讨健脾消积汤联合西药治疗原发性肝癌癌因性疲劳的疗效,以及对患者生存期和生活质量的影响。方法:选择2017年3月~2020年1月唐山市第三医院收治的60例原发性肝癌患者,采用随机数字表法将患者分为观察组和对照组,各30例。两组均采用经导管肝动脉化疗栓塞(TACE)治疗,对照组给予地塞米松和(或)促红细胞生成素治疗12周,观察组在对照组基础上增加健脾消积汤治疗12周。观察两组疗效、生存率和不良反应,比较治疗前后两组疲劳、生活质量差异。结果:观察组CR8例,PR13例,有效率为70.00%,高于对照组的43.33%(P<0.05)。两组治疗后FSS评分、CFS评分、SF-36各项评分和总分均降低(P<0.001),观察组治疗后FSS评分、CFS评分、SF-36各项评分和总分均增加低于对照组(P<0.05)。观察组Ⅲ度及以上不良反应率低于对照组(P<0.05),且观察组生存率为73.33%(22/30),高于对照组的43.33%(13/30)(P<0.05)。结论:健脾消积汤联合西药治疗可提高原发性肝癌治疗效果,降低不良反应,还能缓解患者癌因性疲劳程度,提高患者生活质量,延长近期生存时间。

Objective:To investigate the efficacy of using Jianpi Xiaoji Decoction combined with Western medicine in the treatment of primary hepatocellular carcinoma induced fatigue and its inflence on the patients’survival time and quality of life.Methods:A total of 60 patients with primary hepatocellular carcinoma admitted to the oncology department of our hospital from March 2017 to January 2020 were selected and divided into two groups with 30 patients in each group by random number table method.Both group were treated with Transcatheter Arterial Chemoembolization(TACE),the controlled group was treated with Dexamethasone and/or Erythropoietin for 12 weeks,and the observation group was treated with Jianpi Xiaoji Decoction for 12 weeks on the basis of the controlled group.The efficacy,survival rate and adverse reactions of the two groups were observed,and the differences in fatigue and quality of life between the two groups before and after treatment were compared.Results:The effective rate of CR8 and PR13 in the observation group was 70.00%,higher than 43.33%in the controlled group(P<0.05).FSS scores,CFS scores,SF-36 scores and total scores were all decreased after treatment in both groups(P<0.001),while FSS scores,CFS scores,SF-36 scores and total scores in the observation group were all increased lower than those in the controlled group(P<0.05).Ⅲdegrees and above adverse reaction rate in the observation group was lower than the controlled group(P<0.05),the survival rate was higher than that of controlled group(P<0.05).Conclusion:Jianpi Xiaoji Decoction combined with western medicine can improve the therapeutic effect of primary liver cancer,reduce adverse reactions,relieve the degree of cancer-induced fatigue,improve the quality of life of patients,and prolong the recent survival time.
