参苓白术散加味治疗肺癌根治术后慢性咳嗽临床疗效及对患者炎性因子及免疫功能的影响

Clinical Effect of Using Modified Shenling Baizhu Powder in the Treatment of Chronic Cough after Radical Resection of Lung Cancer and Its Influence on Inflammatory Factors and Immune Function

目的:探讨参苓白术散对治疗肺癌根治术后慢性咳嗽患者的临床疗效、炎性因子及免疫功能的影响。方法:于2018年3月~2020年6月在本院实施肺癌根治术手术治疗,术后出现慢性咳嗽症状的118例患者作为研究对象。根据治疗方法不同分为对照组60例(口服复方甲氧那明胶囊治疗)及观察组58例(参苓白术散汤剂治疗)。对比2组患者临床疗效、炎性因子及免疫因子改善水平,观察不良反应发生率。结果:(1)观察组治疗总有效率93.10%显著高于对照组80.00%(P<0.05)。(2)治疗后,两组IL-4、IL-6、IL-10水平明显下降,IFN-γ水平明显升高(P<0.05),且观察组上述炎性指标改善幅度大于对照组(P<0.05)。(3)治疗后,两组CD_(4)^(+)水平与CD_(4)^(+)/CD_(8)^(+)升高,CD_(8)^(+)水平降低(P<0.05),且观察组上述免疫指标水平改善幅度大于对照组(P<0.05)。(4)2组不良反应总发生率比较,差异无统计学意义(观察组6.90%VS对照组5.00%)(P>0.05)。结论:对肺癌根治术术后慢性咳嗽患者予以参苓白术散汤剂治疗安全性高,可有效缓解慢性咳嗽症状,改善机体炎性反应及免疫功能,临床治疗疗效理想。

Objective:To explore the effect of Shenling Baizhusan Decoction on clinical efficacy,inflammatory factors and immune function for patients with chronic cough after radical resection of lung cancer.Methods:118 patients who were treated with radical resection of lung cancer in the hospital from March 2018 to June 2020 and then had chronic cough symptoms were selected as the research objects.According to the different treatment methods,they were divided into 2 groups,60 cases in the controlled group(treated by oral Compound Methoxyphenamine Capsule)and 58 cases in the observation group(treated by Shenling Baizhusan Decoction).The clinical efficacy and improvement of inflammatory factors and immune factors between the two groups were compared,and the incidence of adverse reactions was observed.Results:(1)The total effective rate of treatment in the observation group was 93.10%,which was significantly higher than 80.00%in the controlled group(P<0.05).(2)After treatment,the levels of IL-4,IL-6,and IL-10 in the two groups significantly decreased,and the level of IFN-γsignificantly increased(P<0.05),and the improvement of the above-mentioned inflammatory indexes in the observation group was greater than that in the controlled group(P<0.05).(3)After treatment,the levels of CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in the two groups increased,and the level of CD_(8)^(+)decreased(P<0.05),and the improvement of the above-mentioned immune index levels in the observation group was greater than that in the controlled group(P<0.05).(4)There was no statistically significant difference in the total incidence of adverse reactions between the two groups(6.90%in the observation group VS 5.00%in the controlled group)(P>0.05).Conclusion:In the treatment of patients with chronic cough after radical resection of lung cancer,Shenling Baizhusan Decoction has high safety,can effectively relieve chronic cough symptoms,improve the body’s inflammatory response and immune function,and has an ideal clinical treatment effect.
