通络消痹汤足浴联合穴位贴敷治疗寒凝血瘀型糖尿病周围神经病变临床观察

Clinical Observation of Applying Tongluo Anti-paralysis Decoction Foot Bath Combined with Acupuncture Point Application in the Treatment of Cold-coagulation and Blood-stasis Diabetic Peripheral Neuropathy Patients

目的:探讨寒凝血瘀型糖尿病周围神经病变患者行通络消痹汤足浴和穴位贴敷联合治疗的临床效果。方法:选择我院2019年2月~2021年6月住院部收治的108例寒凝血瘀型糖尿病周围神经病变患者,根据治疗方式的不同分为54例对照组:予常规控制血糖、甲钴胺片治疗;54例观察组:在对照组基础上加用通络消痹汤足浴联合穴位贴敷治疗。系统比较两组患者临床疗效、中医证候积分、多伦多临床评分系统(TCSS)评分、神经传导速度。结果:(1)治疗后,观察组总有效率为85.19%,显著高于对照组总有效率68.52%(P<0.05);(2)两组患者中医证候总积分均较前显著改善(P<0.05),且观察组中医证候积分显著优于对照组(P<0.05);(3)两组患者TCSS评分均较前显著改善(P<0.05),且观察组TCSS评分显著优于对照组(P<0.05);(4)两组神经传导速度较前均明显加快(P<0.05),且观察组神经传导速度更加显著(P<0.05)。结论:通络消痹汤足浴和穴位贴敷联合治疗寒凝血瘀型糖尿病周围神经病变患者,能够有效改善其周围神经病变,值得推广应用。

Objective:To investigate the clinical effect of using Tongluo Xiaobi Decoction combined with acupoint application in treating diabetic peripheral neuropathy with cold coagulation and blood stasis.Medthods:108 cases of diabetic peripheral neuropathy with cold coagulation and blood stasis admitted in our hospital from February 2019 to June 2021 were selected and divided into the controlled groups with 54 cases according to the different treatment methods:routine blood glucose control and Mecobalamin Tablets treatment.54 cases in the observation group:Tongluo Xiaobi Decoction foot bath combined with acupoint application was added to the controlled group.The clinical efficacy,TCM syndrome score,Toronto clinical scoring system(TCSs)score and nerve conduction velocity were systematically compared between the two groups.Results:(1)After treatment,the total effective rate in the observation group was 85.19%,which was significantly higher than 68.52%in the controlled group(P<0.05);(2)The total score of TCM syndrome in the two groups was significantly improved(P<0.05),and the score of TCM syndrome in the observation group was significantly better than that in the controlled group(P<0.05);(3)The TCSs scores of the two groups were significantly improved(P<0.05),and the TCSs scores of the observation group were significantly better than those of the controlled group(P<0.05);(4)The nerve conduction velocity in the two groups was significantly faster than that in the previous group(P<0.05),and the nerve conduction velocity in the observation group was more significant(P<0.05).Conclusion:Tongluo Xiaobi Decoction combined with acupoint application can effectively improve peripheral neuropathy in patients with diabetic peripheral neuropathy,and is worthy of popularization and application.
