和胃通泻合剂治疗急性胰腺炎临床疗效观察

Clinical Efficiency of Using Hewei Tongxie Decoction in the Treatment of Acute Pancreatitis Patients

目的:观察和胃通泻合剂对重症急性胰腺炎的临床疗效,为中药治疗急性胰腺炎提供临床依据。方法:将68例符合纳入标准的重症急性胰腺炎患者按照随机数字表法完全随机分为对照组、观察组,每组34例。对照组给予常规西医治疗,观察组在常规西医治疗的基础上联合和胃通泻合剂口服加灌肠。观察治疗前、治疗3天及7天后的临床症状和体征(腹胀腹痛、排气排便、肠鸣音)、血淀粉酶、尿淀粉酶、CRP、Ca^(2+)、Marshall评分、APACHEH II评分等的变化。结果:经过7天的治疗后,观察组总有效率为94.12%,对照组总有效率为82.35%,差异有统计学意义(P<0.05);观察组的腹胀腹痛、排气排便、肠鸣音、血淀粉酶、尿淀粉酶、Marshall、APACHEH II等较对照组明显改善,差异有统计学意义(P<0.05);但是观察组和对照组的CRP比较差异无统计学意义。结论:和胃通泻合剂能够明显提常规西医治疗高重症急性胰腺炎的有效率,缓解重症急性胰腺炎的症状、体征和生化指标,促进胃肠功能恢复。

Objective:To observe the clinical efficiency of Hewei Tongxie Decoction on treating severe acute pancreatitis(SAP),so as to provide clinical basis for treating acute pancreatitis with traditional Chinese medicine.Methods:68 patients with severe acute pancreatitis were randomly divided into a controlled group and an observation group,with 34 cases in each group.The controlled group was treated with conventional western medicine,while the observation group was treated with Hewei Tongxie Decoction orally and enema on the basis of conventional western medicine.The changes of clinical symptoms,signs(abdominal distension and pain,exhaust defecation,bowel sounds),blood amylase,urine amylase,CRP,Ca^(2+),Marshall and APACHEH II were observed before treatment and 3 days and 7 days after treatment.Results:After 7 days of treatment,the total efficacy was 94.12%in the observation group and 82.35%in the controlled group(P<0.05).The abdominal distension,abdominal pain,exhaust defecation,bowel sounds,blood amylase,urine amylase,Marshall and APACHEH II in the observation group were significantly improved compared with those in the controlled group(P<0.05).However,there was no significant difference in CRP between the observation group and the controlled group after 7 days treatment.Conclusion:Hewei Tongxie Decoction can improve the efficacy of conventional western medicine treatment in SAP patients,relieving the symptoms,signs and biochemical indicators,thus promoting the recovery of gastrointestinal function.
