便秘推拿联合穴位按摩对老年气虚血瘀证胸痹合并便秘患者护理观察

Nursing Observation of Constipation Massage Combined with Acupoint Massage for Elderly Patients with Qi-deficiency and Blood-stasis Syndrome of Thoracic Obstruction and Constipation

目的:探究便秘推拿联合穴位按摩对老年气虚血瘀证胸痹合并便秘患者的护理效果。方法:选取2018年9月~2019年12月在本院接受治疗的老年气虚血瘀证胸痹合并便秘患者148例,随机数表法将所有患者分为两组,观察组和对照组均74例患者。其中观察组患者采用便秘推拿联合穴位按摩护理,对照组患者仅采用便秘推拿护理。对比两组患者便秘治疗疗效,护理前后,采用Wexner便秘评分表评定两组患者的便秘情况,生活质量综合评定问卷(GQOL-74)测评两组患者生活质量水平,焦虑自评量表评分(SAS)、抑郁自评量表评分(SDS)评分测评患者的心理状态变化。结果:观察组便秘治疗总有效率为89.04%,高于对照组的58.46%(P<0.05);两组患者护理后的Wexner、SAS、SDS评分均降低(P<0.05),且观察组护理后的上述参数均小于对照组(P<0.05);两组患者各项生活质量因子评分均升高(P<0.05),且观察组护理后参数水平均大于对照组(P<0.05)。结论:便秘推拿联合穴位按摩能有效缓解老年气虚血瘀证胸痹合并便秘患者症状,提高患者的生活质量,临床可根据患者的具体情况参考采用。

Objective:This study was aimed to explore the nursing effect of massage for constipation combined with acupoint massage for elderly patients with Qi-deficiency and blood-stasis syndrome of thoracic obstruction and constipation.Methods:Between September 2018 and December 2019,148 elderly patients with Qi-deficiency and blood-stasis syndrome of thoracic obstruction and constipation treated in our hospital were selected and randomly divided into 2 groups:an observation group(74 cases)and a controlled group(74 cases).Patients in the observation group received massage for constipation combined with acupoint massage,and those in the controlled group only received massage for constipation.Therapeutic effects on constipation were compared between the 2 groups.The Wexner scale was used to assess the situation of constipation before and after nursing,and the comprehensive quality of life assessment questionnaire(GQOL-74)(physical function,psychological function,social function,material life scores)was used to evaluate quality of life before and after nursing.Also,the self-rating anxiety scale(SAS)and the self-rating depression scale(SDS)scores before and after nursing were recorded.Results:The total response rate in the observation group was 89.04%,higher than 58.46%in the controlled group(P<0.05).After nursing,the Wexner,SAS,and SDS scores of both groups decreased(P<0.05),and the above scores of the observation group were lower than those of the controlled group(P<0.05).Quality of life scores increased(P<0.05),and the observation group had higher scores than the controlled group(P<0.05).Conclusion:Massage for constipation combined with acupoint massage can effectively relieve symptoms of elderly patients with Qi-deficiency and blood-stasis syndrome of thoracic obstruction and constipation,and improve their quality of life.In clinical practice,the nursing method can be adopted according to the specific condition.
