旋复花汤合温胆汤加减联合瑞巴派特片治疗慢性萎缩性胃炎临床疗效观察

Clinical Efficacy of Using Modified Xuanfuhua Decoction and Wendan Decoction Combined with Rebamipide Tablets in the Treatment of Patients with Chronic Atrophic Gastritis

目的:探讨旋复花汤合温胆汤加减联合瑞巴派特片治疗慢性萎缩性胃炎(CAG)患者的临床疗效。方法:选取2018年1月~2020年1月正宁县人民医院收治的96例CAG患者,根据数字表法均分为两组,对照组接受瑞巴派特片治疗,观察组接受旋复花汤合温胆汤加减联合瑞巴派特片治疗,对比两组患者临床疗效和治疗前后中医证候积分、病理积分、胃功能指标及不良反应发生情况。结果:观察组治疗总有效率明显高于对照组(P<0.05);治疗后观察组中医证候积分和异型增生、肠上皮化生、腺体萎缩积分及血清内皮素-1(ET-1)、表皮生长因子(EGF)水平明显低于对照组,血清胃蛋白酶原Ⅰ(PGⅠ)、胃泌素-17(G-17)水平明显高于对照组(P<0.05);两组患者不良反应发生率对比无差异(P>0.05)。结论:旋复花汤合温胆汤加减联合瑞巴派特片治疗CAG临床疗效确切,可有效改善临床症状,可能与其调节ET-1、EGF、PGⅠ、G-17表达有关,且不会降低治疗安全性,值得推广。

Objective:To investigate the clinical efficacy of using Modified Xuanfuhua Decoction and Wendan Decoction combined with Rebamipide Tablets in the treatment of patients with chronic atrophic gastritis(CAG).Methods:A total of 96 CAG patients admitted to our hospital(January 2018 to January 2020)were selected and divided into two groups according to the random number table method.The controlled group(n=48)received Rebamipide Tablets,and the observation group(n=48)after receiving Modified Xuanfuhua Decoction and Wendan Decoction combined with Rebamipide Tablets,the clinical efficacy and TCM syndrome scores,pathological scores,gastric function indicators and adverse reactions were compared between the two groups of patients before and after treatment.Results:The total effective rate of treatment in the observation group was significantly higher than that in the controlled group(P<0.05);after treatment,the observation group’s TCM syndrome score and dysplasia,intestinal metaplasia,gland atrophy score and serum endothelin-1(ET-1)and epidermal growth factor(EGF)levels were significantly lower than those of the controlled group,and the levels of serum pepsinogenⅠ(PGⅠ)and gastrin-17(G-17)were significantly higher than those of the controlled group(P<0.05);there was no difference in the incidence of reaction(P>0.05).Conclusion:Modified Xuanfuhua Decoction and Wendan Decoction combined with Rebamipide Tablets have a definite clinical effect in the treatment of CAG,which can effectively improve clinical symptoms,which may be related to the regulation of ET-1,EGF,PGⅠ,and G-17 expression,and will not decrease the safety of treatment is worthy of promotion.
