活络沐足散中药熏洗联合保髋手术对股骨头坏死血瘀证患者的干预研究

Intervention Study of Huoluo Muzu Powder Chinese Herbal Fumigation and Washing Combined with Hip Preserving Surgery for Patients with Femoral Head Necrosis and Blood Stasis

目的:观察活络沐足散中药熏洗联合保髋手术对股骨头坏死(ONFH)血瘀证患者的干预效果。方法:将我院2019年4月至2021年1月42例ONFH血瘀证患者随机分为试验组和对照组,各21例。对照组患者采用手术治疗,试验组在对照组基础上,给予患者活络沐足散中药熏洗治疗。比较两组患者的临床疗效、髋关节功能、疼痛、生活质量、凝血功能。结果:试验组总有效率为90.47%,对照组为76.19%,试验组患者总有效率明显高于对照组(P<0.05)。治疗前两组患者髋关节功能、疼痛及生活质量评分比较无显著性差异(P>0.05),治疗1个月后,两组患者髋关节功能、生活质量评分均增高,疼痛评分下降;试验组患者髋关节功能、生活质量评分高于对照组(P<0.05),疼痛评分低于对照组(P<0.05)。治疗前两组患者凝血酶时间(TT)、凝血酶原时间(PT)、部分凝血活酶时间(APTT)、纤维蛋白(FIB)水平无明显差异(均为P>0.05),治疗1个月后两组患者的PT、APTT、FIB水平下降,试验组患者的PT、APTT、FIB水平均低于照组(均为P<0.05)。TT在两组间的比较上未见明显统计学意义。结论:活络沐足散中药熏洗联合保髋手术对ONFH血瘀证患者的干预效果较好,改善髋关节功能和凝血功能,降低疼痛,提高生活质量。

Objective:To observe the intervention effect of Huoluo Muzu Powder traditional Chinese medicine fumigation and washing combined with hip preservation surgery for patients with blood stasis syndrome of femoral head necrosis(ONFH).Methods:From April 2019 to January 2021,42 patients with ONFH blood stasis syndrome were randomly divided into an experimental group and a controlled group,with 21 cases in each group.Patients in the controlled group were treated with surgery,and the experimental group was treated with Huoluo Muzu Powder Chinese medicine fumigation and washing treatment on the basis of the controlled group.The clinical efficacy,hip joint function,pain,quality of life,and blood coagulation function were compared between the two groups.Results:The total effective rate of the experimental group was 90.47%,and that of the controlled group was 76.19%.The total effective rate of the experimental group was significantly higher than that of the controlled group(P<0.05).Before treatment,there was no significant difference in hip function,pain and quality of life scores between the two groups of patients(P>0.05).After 1 month of treatment,the hip function and quality of life scores of the two groups increased,while the pain scores decreased.The scores of the experimental group of hip joint function and quality of life were higher than those of the controlled group(P<0.05),and the pain scores were lower than those of the controlled group(P<0.05).Before treatment,there was no significant difference in thrombin time(TT),prothrombin time(PT),partial thromboplastin time(APTT),and fibrin(FIB)levels between the two groups of patients(all P>0.05),treatment for 1 month.The levels of PT,APTT,and FIB of the latter two groups decreased,and the levels of PT,APTT,and FIB of the experimental group were lower than those of the controlled group(all P<0.05).There was no significant statistical significance in the comparison of TT between the two groups.Conclusion:Huoluo Muzu Powder Chinese medicine fumigation and washing combined with hip-preserving surgery has a better intervention effect for patients with blood stasis syndrome of ONFH,improving hip joint function and blood coagulation function,reducing pain and improving quality of life.
