中医护理干预联合吴茱萸穴位贴敷对腹腔镜胆总管结石手术患者胃肠道功能的影响

Effect of Traditional Chinese Medicine Nursing Intervention Combined with Acupoint Application of Fructus Evodiae for Patients with Laparoscopic Choledocholithiasis Surgery

目的:研究中医护理干预联合吴茱萸穴位贴敷对腹腔镜胆总管结石手术患者的临床效果以及满意度影响。方法:选2019年1月~2020年12月期间本院肝胆科收治行腹腔镜胆总管手术80例,采用随机数字表法分为两组,对照组40例(中医护理干预)和观察组40例(加用吴茱萸穴位冷敷),比较两组患者术后肠鸣音恢复时间、首次下床活动时间、术后首次排气时间、术后住院天数,测定胃肠道功能的变化,评估患者对护理效果的满意度。结果:观察组患者肠鸣音恢复时间、首次下床活动时间、首次排气时间、术后住院天数短于对照组(P<0.05);观察组患者胃动素、胃泌素水平高于对照组(P<0.05);观察组患者中医症状积分降低幅度高于对照组(P<0.05);观察组护理效果满意率为92.50%%,显著高于对照组(P<0.05)。结论:中医护理干预联合吴茱萸中药贴敷可促进术后患者胃肠功能恢复,提高腹腔镜胆总管患者术后舒适度,促进术后恢复,减轻患者疼痛,具有一定的临床应用价值。

Objective:To study the influence of traditional Chinese medicine nursing intervention combined with acupoint application of fructus evodiae on the clinical effect and satisfaction for patients with laparoscopic choledocholithiasis surgery.Methods:80 cases of laparoscopic common bile duct surgery in the hepatobiliary department of our hospital from January 2019 to December 2020 were randomly divided into two groups,40 cases in the controlled group(traditional Chinese medicine nursing intervention)and 40 cases in the observation group(additional cold compress application of Fructus Evodiae).The postoperative bowel sound recovery time,postoperative first ambulation time,first postoperative exhaust time and postoperative hospital stay were compared between the two groups.The changes in gastrointestinal function were measured,and the patient’s satisfaction with nursing effect was evaluated.Results:The bowel sound recovery time,first ambulation time,first exhaust time and postoperative hospital stay in the observation group were shorter than those in the controlled group(P<0.05).The levels of motilin and gastrin were higher in the observation group than those in the controlled group(P<0.05).The reductions of TCM symptoms in the observation group were higher compared with those in the controlled group(P<0.05).The satisfaction rate of nursing effect with 97.62%in the observation group was significantly higher than that in the controlled group(P<0.05).Conclusion:Traditional Chinese medicine nursing intervention combined with acupoint application of Fructus Evodiae can promote the recovery of postoperative gastrointestinal function,improve the postoperative comfort,promote the postoperative recovery and relieve the pain of patients with laparoscopic choledocholithiasis surgery,and it has certain clinical application value.
