附子大黄细辛汤加味辅助盐酸米诺环素治疗对老年慢性牙周炎患者牙周状态及龈沟液炎症指标的影响

Effect of Fuzi Dahuang Xixin Decoction Supplemented with Minocycline Hydrochloride on Periodontal Status and Gingival Crevicular Fluid Inflammatory Indicators for Elderly Patients with Chronic Periodontitis

目的:探讨附子大黄细辛汤加味辅助盐酸米诺环素治疗对老年慢性牙周炎患者牙周状态及龈沟液炎症指标的影响。方法:选取我院于2018年3月~2020年4月间收治的93例老年慢性牙周炎患者,按随机数字表法分为研究组(n=46)和对照组(n=47)。评价两组患者治疗的总有效率,记录并比较治疗前和治疗6周后两组患者的牙菌斑指数(PLI)和牙龈出血指数(GBI)、牙周探诊深度(PD)。检测两组患者龈沟液中白细胞介素6(IL-6)、白介素-1β(IL-1β)、超敏C反应蛋白(hs-CRP)水平。结果:治疗前两组患者的PLI、GBI、PD评分和龈沟液中IL-6、IL-1β、hs-CRP水平组间相比无明显差异(P>0.05)。治疗后,两组患者的PLI、GBI、PD评分和龈沟液中IL-6、IL-1β、hs-CRP水平均较治疗前明显降低(P<0.05)。且研究组患者总有效率明显高于对照组(P<0.05),PLI、GBI、PD评分明显降于对照组(P<0.05),龈沟液中IL-6、IL-1β、hs-CRP水平均明显低于对照组(P<0.05)。结论:附子大黄细辛汤加味辅助盐酸米诺环素治疗老年慢性牙周炎患者的疗效显著,可改善患者牙周状态,减轻龈沟液炎症。

Objective:To investigate the effect of Fuzi Dahuang Xixin Decoction supplemented with minocycline hydrochloride on the periodontal status and gingival crevicular fluid inflammatory indicators for elderly patients with chronic periodontitis.Methods:A total of 93 elderly patients with chronic periodontitis admitted to the hospital were selected between March 2018 and April 2020,and they were divided into a study group(n=46)and a controlled group(n=47)according to the random number table method.The total effective rate of treatment was evaluated in the two groups of patients,and the plaque index(PLI),gingival bleeding index(GBI)and periodontal probing depth(PD)of the two groups were recorded and compared before treatment and after 6 weeks of treatment.The levels of interleukin-6(IL-6),interleukin-1β(IL-1β)and high-sensitivity C-reactive protein(hs-CRP)in the gingival crevicular fluid of the two groups of patients were detected.Results:Before treatment,there were no significant differences in PLI,GBI,PD and levels of gingival crevicular fluid IL-6,IL-1β and hs-CRP between the two groups(P>0.05).After treatment,the PLI,GBI,PD and levels of IL-6,IL-1β and hs-CRP in gingival crevicular fluid of the two groups were significantly decreased compared with those before treatment(P<0.05).The total effective rate of patients in the study group was significantly higher than that in the controlled group(P<0.05)while the PLI,GBI,and PD were significantly lower than those in the controlled group(P<0.05),and the levels of IL-6,IL-1β and hs-CRP in the gingival crevicular fluid were significantly lower than those in the controlled group(P<0.05).Conclusion:Fuzi Dahuang Xixin Decoction supplemented with minocycline hydrochloride has a significant efficacy in the treatment of elderly patients with chronic periodontitis,and it can improve the periodontal status and reduce the gingival crevicular fluid inflammation.
