自拟养血润燥熄风汤治疗鱼鳞病(血虚风燥证)临床观察

Clinical Effect of Using Self-made Yangxue Runzao Xifeng Decoction in the Treatment of Ichthyosis with Blood-deficiency and Wind-dryness Syndrome

目的:观察自拟养血润燥熄风汤治疗血虚风燥证鱼鳞病的临床疗效。方法:选取本院于2017年3月至2019年12月收治的血虚风燥证鱼鳞病患者64例,采用随机数表法将其分为对照组与治疗组,各32例。对照组予以维A酸乳膏外用涂擦治疗,治疗组在对照组的基础上给予养血润燥熄风汤治疗,两组均治疗8周。评价两组的疗效,并对比两组治疗前后临床症状、皮肤屏障功能、免疫功能以及炎性因子各指标水平变化情况。结果:两组的临床有效率分别为93.75%和75.00%,治疗组高于对照组(P<0.05)。8周后治疗组的瘙痒、皮损面积及皮疹形态评分均低于对照组(P<0.05)。8周后,两组的皮脂含量及角质层含水量较治疗前提高,经表皮水分丢失较治疗前下降(P<0.05),且治疗组变化幅度大于对照组(P<0.05)。8周后,治疗组的CD4^(+)、CD4+/CD8_(+)、IL-2、IL-8及IFN-γ水平较对照组更高,CD8^(+)及IL-4水平较对照组更低(P<0.05)。结论:自拟养血润燥熄风汤治疗血虚风燥证鱼鳞病,可明显改善患者临床症状,提高其皮肤屏障及免疫功能,降低炎性因子反应水平,效果显著。

Objective:To investigate the clinical effect of using Self-made Yangxue Runzao Xifeng Decoction in the treatment of ichthyosis with blood-deficiency and wind-dryness syndrome.Methods:A total of 64 patients with ichthyosis of blood-deficiency and wind-dryness syndrome admitted to the hospital from March 2017 to December 2019 were selected as study subjects.They were divided into a controlled group and a treatment group by the random number table method,with 32 cases in each group.The controlled group received external application of tretinoin cream,and on this basis,the treatment group were treated with Yangxue Runzao Xifeng Decoction.Both groups underwent 8 weeks of treatment,and therapeutic effects of the two groups were evaluated.The changes in clinical symptoms,skin barrier function,immune function and inflammatory factor levels before and after treatment were compared between the two groups.Results:The clinical effective rate in the treatment group was 93.75%,higher than 75.00%in the controlled group(P<0.05).Eight weeks later,the scores for itching,skin lesion area and rash morphology of the treatment group were lower than those of the controlled group(P<0.05).In the two groups,the sebum content and stratum corneum water content increased,and the transepidermal water loss decreased after treatment(P<0.05).The changes in the treatment group were greater than those in the controlled group(P<0.05).Eight weeks later,the levels of CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),IL-2,IL-8 and IFN-γ in the treatment group were higher than those in the controlled group,and the levels of CD_(8)^(+)and IL-4 were lower than those in the controlled group(P<0.05).Conclusion:Self-made Yangxue Runzao Xifeng Decoction is applied in the treatment of ichthyosis with blood-deficiency and wind-dryness syndrome can significantly alleviate the patients’clinical symptoms,improve their skin barrier function and immune function,and reduce the levels of inflammatory factors.
