麻醉全程镇痛管理对产妇分娩镇痛质量及泌乳功能的效果研究

The effect of anesthesia on the quality of the analgesic andthe function of lactation

目的探讨麻醉全程镇痛管理对产妇分娩镇痛质量及泌乳功能的效果。方法选择2020年1月至2022年5月衡水市妇幼保健院行硬膜外分娩镇痛的足月初产妇205例为研究对象,随机数字表法分成对照组(n=102)和观察组(n=103)。对照组分娩镇痛时给予常规产科护理;观察组由麻醉师对产妇进行全程镇痛管理及健康教育。观察两组产妇分娩镇痛质量及泌乳功能。结果观察组宫口开全时、分娩期最高VAS评分低于对照组(P<0.05);观察组分娩期间VAS评分≥7分产妇比例低于对照组(P<0.05)。观察组泌乳始动时间短于对照组,产后24 h、产后48 h血浆PRL浓度高于对照组(P<0.05);2组产后24 h、产后48 h血浆PRL浓度均高于产前(P<0.05)。结论在分娩镇痛过程中,麻醉师全程镇痛管理,提高了镇痛质量,促进泌乳功能的提高,值得推行。

Objective:To explore the effects of anesthesia analgesia management on the quality of labor analgesia and lactation function.Methods:A total of 205 full-term primipara who underwent epidural labor analgesia in Hengshui Maternal and Child Health Hospital from January 2020 to May 2022 were selected as the study objects,and were divided into control group(n=102)and observation group(n=103)by random number table method.The control group was given routine obstetric care during labor analgesia;In the observation group,anesthesiologists carried out analgesic management and health education.The analgesic quality and lactation function of the two groups were observed.Results:The maximum VAS score of the observation group was lower than that of the control group(P<0.05).The proportion of pregnant women with VAS score≥7 in observation group was lower than that in control group(P<0.05).The start time of lactation in the observation group was shorter than that in the control group,and the plasma PRL concentration at 24 h and 48 h postpartum was higher than that in the control group(P<0.05).The plasma PRL concentration at 24 h and 48 h postpartum in 2 groups was higher than that before(P<0.05).Conclusion:In the process of labor analgesia,anesthesiologist analgesia management can improve the quality of analgesia and promote the improvement of lactation function,which is worth implementing.
