摘要
脑缺血再灌注损伤(cerebral ischemia-reperfusion injury,CIRI)是缺血性卒中血管再通的常见病理损伤,可加重神经功能缺损及大脑功能障碍。p38MAPK通路与CIRI病理机制密切相关,可通过促进炎症反应、抑制氧化应激、调控细胞凋亡等参与缺血性脑卒中的损伤机制,影响脑卒中的疾病进展。中医认为,缺血性中风的主要病机为气虚血瘀、痰瘀互阻,治疗当以益气活血、通脉祛浊为主。从p38MAPK分子生物功能出发,结合脑缺血病理生理机制、中医病机及中医药现代研究成果,全方位、多角度探讨中医药防治脑缺血,可为脑缺血的治疗及靶点研究提供参考。
Cerebral ischemia-reperfusion injury(CIRI)is a common pathological injury of revascularization in ischemic stroke which aggravates nerve function defect and brain function disorder.p38MAPK pathway is closely related to the pathological mechanism of CIRI and plays a role in the injury mechanism of ischemic stroke and the progression of stroke by promoting inflammatory response,inhibiting oxidative stress and regulating apoptosis.In the field of the theory of traditional Chinese medicine(TCM),the main pathogenesis of ischemic stroke is due to qi deficiency and blood stasis,and the obstruction of both phlegm and blood stasis,and then the treatment should focus on replenishing qi and activating blood,dredging vessels and removing turbidity.Based on the molecular biological function of p38MAPK and by combining the achievement in the research of pathophysiological mechanism of cerebral ischemia,TCM pathogenesis and modern practice with TCM,the exploration on the prevention and treatment of cerebral ischemia with TCM in a comprehensive and multi-level approach will provide reference for the treatment and target research of cerebral ischemia.
