青黛古今临床应用的对比分析

Comparative Analysis of the Ancient and Modern Clinical Applications of Indigo Naturalis

【目的】结合青黛适用的专科病种情况及临床用药现状,比较其古今临床应用情况的异同,为青黛的现代开发提供线索。【方法】系统检索《中医方剂大辞典》以及中国生物医学文献数据库(SinoMed)、万方数据知识服务平台(Wanfang Data)、中国知网数据库(CNKI)及重庆维普数据库(VIP),筛选含青黛的古方及其今方的临床研究文献,建立青黛古今临床应用适应证数据库。对数据库中含青黛方剂的所有适应证的临床科属及病种进行提取和分析,比较其临床应用领域与病种。【结果】共纳入含青黛的古方1323首,有关青黛及其制剂的现代临床研究文献2633篇。含青黛的古代载方按病种拆分为1935条记录,现代文献按照方剂及病种拆分为2813条记录。古代文献中青黛的使用涉及儿科、五官科、皮肤科、呼吸科、消化科等疾病。现代文献中青黛的使用涉及消化科、皮肤科、五官科、血液科、感染科等疾病;高频病种的频次由高到低依次为溃疡性结肠炎、银屑病、口腔溃疡、带状疱疹、白血病等。【结论】古今文献中青黛应用的专科疾病多集中在五官科、皮肤科、消化科、感染科,病种主要为炎性疾病;青黛的现代应用在剂型、给药方式上有所优化。治疗白血病是青黛的现代新用,含青黛的中成药治疗白血病的机制阐述已获得国际药学界认可。青黛临床应用的今后探索需结合制药、药理等多个学科领域的研究成果,根据前人的经验拓展新的适用病种,并科学地验证其有效性与安全性。

Objective The similarities and differences in the ancient and modern clinical applications of Indigo Naturalis were compared based on the indicated specialist diseases and clinical medication methods of Indigo Naturalis,so as to provide clues for its modern development.Methods A systematic retrieval of the Dictionary of Traditional Chinese Medicine Prescriptions and the Chinese databases of SinoMed,Wanfang Data,CNKI,and VIP was conducted to select ancient formulas containing Indigo Naturalis and modern clinical research literature of Indigo Naturalis,and then the database of indications for the ancient and modern clinical applications of Indigo Naturalis was established.The clinical classification and disease categories of all the indications in the database were extracted and analyzed to compare the similarities and differences in the clinical applications and disease categories.Results A total of 1323 ancient prescriptions containing Indigo Naturalis were included,and2633 modern clinical research literature related to Indigo Naturalis and its preparations were enrolled.And a total of 1935 records of ancient prescriptions containing Indigo Naturalis were screened out by disease categories,and 2813 records of the modern literature were screened out by prescription types and disease categories.Indigo Naturalis recorded in the ancient literature was indicated for the diseases associated with pediatrics,ophthalmology and otorhinolaryngology,dermatology,respiratory diseases,digestive diseases and other clinical specialties.Indigo Naturalis recorded in the modern literature was indicated for the diseases associated with digestive diseases,dermatology,ophthalmology and otorhinolaryngology,hematology and infectious diseases.The high-frequency diseases in descending order were ulcerative colitis,psoriasis,oral ulcers,herpes zoster and leukemia.Conclusion The indications of Indigo Naturalis recorded in ancient and modern literature involve the diseases of ophthalmology and otorhinolaryngology,dermatology,digestive diseases and infectious diseases,and the categories of the diseases are usually shown as inflammatory diseases.The dosage form and administration methods of Indigo Naturalis in modern literature have been optimized to certain degree.Leukemia is the new modern indication of Indigo Naturalis,and the achievements in the therapeutic mechanism of Chinese patent medicine containing Indigo Naturalis for the treatment of leukemia have been recognized by the international pharmacological experts.The future exploration of clinical application of Indigo Naturalis should be based on the achievements in multiple disciplinary fields such as pharmacy and pharmacology,and can focus on expanding new indications based on previous experience,and verifying its effectiveness and safety scientifically.
