温通膏联合玻璃酸钠注射治疗寒湿痹阻型肩关节周围炎疗效研究

Clinical Effect of Wentong Ointment Combined with Sodium Hyaluronate Injection for the Treatment of Scapulohumeral Periarthritis with Cold-damp Obstruction Syndrome

【目的】观察温通膏联合玻璃酸钠注射治疗寒湿痹阻型肩关节周围炎(简称肩周炎)的治疗效果。【方法】将60例寒湿痹阻型肩周炎患者随机分为治疗组和对照组,每组各30例。2组患者均给予肩关节功能锻炼的基础治疗,在此基础上,对照组给予玻璃酸钠肩关节注射治疗,治疗组给予温通膏外涂联合玻璃酸钠肩关节注射治疗,疗程为5周。观察2组患者治疗前后肩关节疼痛视觉模拟量表(VAS)评分、Constant-Murley肩关节功能评分及日常生活能力(ADL)评分的变化情况,并评价2组患者的临床疗效。【结果】(1)治疗5周后,治疗组的总有效率为93.33%(28/30),对照组为83.33%(25/30),组间比较,治疗组的疗效明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗后,2组患者的肩关节疼痛VAS评分均较治疗前明显降低(P<0.01),Constant-Murley肩关节功能评分和ADL评分均较治疗前明显提高(P<0.01),且治疗组对肩关节疼痛VAS评分的降低作用及对Constant-Murley肩关节功能评分和ADL评分的提高作用均明显优于对照组,差异均有统计学意义(P<0.05)。【结论】温通膏联合玻璃酸钠注射治疗寒湿痹阻型肩周炎患者,可有效改善关节疼痛和功能活动情况,临床疗效满意,具有较高的临床应用价值。

Objective To observe the therapeutic effect of Wentong Ointment combined with sodium hyaluronate injection in treating cold-damp obstruction type of scapulohumeral periarthritis of shoulder(also known as frozen shoulder).Methods Sixty patients with cold-damp obstruction type of frozen shoulder were randomly divided into the treatment group and the control group,30 patients in each group.Both groups were required to have shoulder function exercises,and additionally,the control group was given shoulder intra-articular injection of sodium hyaluronate injection,and the treatment group was given Wentong Ointment combined with sodium hyaluronate injection,and the course of treatment lasted for 5 weeks.The visual analogue scale(VAS)scores of shoulder pain,Constant-Murley scores of shoulder function and activities of daily life(ADL)scores in the two groups were observed before and after treatment,and the clinical efficacy of the two groups was evaluated after treatment.Results(1)After 5 weeks of treatment,the overall effective rate of the treatment group was 93.33%(28/30)and that of the control group was 83.33%(25/30),and the intergroup comparison showed that the therapeutic effect of the treatment group was significantly superior to that of the control group,the difference being statistically significant(P<0.05).(2)After treatment,the VAS scores of shoulder pain in both groups were significantly decreased and the Constant-Murley shoulder function scores and ADL scores were significantly increased compared with those before treatment(P<0.01),and the effect on lowering the VAS scores of shoulder pain and on improving the Constant-Murley shoulder function scores and ADL scores in the treatment group was superior to that in the control group,the difference being statistically significant(P<0.05).Conclusion For the treatment of patients with cold-damp obstruction type of frozen shoulder,Wentong Ointment combined with sodium hyaluronate injection can effectively relieve joint pain and improve the should function,and can obtain satisfactory clinical efficacy with good clinical application prospect.
