消癥活血益肾方联合炔雌醇环丙孕酮片治疗肾虚血瘀型多囊卵巢综合征所致不孕症临床研究

Clinical Study on Xiaozheng Huoxue Yishen Prescription Combined with Ethinyl Estradiol Cyproterone Tablets for Infertility Due to Polycystic Ovary Syndrome of Kidney Deficiency and Blood Stasis Type

目的:观察消癥活血益肾方联合炔雌醇环丙孕酮片治疗肾虚血瘀型多囊卵巢综合征所致不孕症的临床疗效及对性激素、血液流变学的影响。方法:采用随机数字表法将124例多囊卵巢综合征所致不孕症患者分为对照组与治疗组各62例。对照组给予炔雌醇环丙孕酮片治疗,治疗组在对照组基础上加用消癥活血益肾方治疗。比较2组治疗前后中医证候积分、血清性激素、血液流变学指标;比较2组临床疗效、排卵率、妊娠率、不良反应发生率。结果:治疗组总有效率、排卵率、妊娠率均高于对照组(P<0.05)。治疗后,2组中医证候积分、全血低切黏度、全血高切黏度、血浆黏度水平均较治疗前降低(P<0.05),且治疗组治疗后中医证候积分、血液流变学指标低于对照组(P<0.05)。治疗后,2组血清睾酮(T)、卵泡刺激素(FSH)、促黄体生成素(LH)、硫酸脱氢表雄酮(DHEAS)水平均较治疗前降低(P<0.05),性激素结合球蛋白(SHBG)水平升高(P<0.05),且治疗组治疗后T、FSH、LH、DHEAS水平低于对照组(P<0.05),SHBG水平高于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:消癥活血益肾方联合炔雌醇环丙孕酮片治疗肾虚血瘀型多囊卵巢综合征所致不孕症疗效确切,能够有效提高患者排卵率、妊娠率,调节患者性激素水平,改善血液流变学异常状态,安全性较高。

Objective:To observe the clinical effect of Xiaozheng Huoxue Yishen Prescription combined with Ethinyl Estradiol Cyproterone Tablets for infertility due to polycystic ovary syndrome of kidney deficiency and blood stasis type and its effect on sex hormones and hemorrheology.Methods:A total of 124 cases of patients with infertility due to polycystic ovary syndrome were divided into the control group and the treatment group according to the random number table method,with 62 cases in each group.The control group was treated with Ethinyl Estradiol Cyproterone Tablets,and the treatment group was additionally treated with Xiaozheng Huoxue Yishen Prescription based on the treatment of the control group.Before and after treatment,the traditional Chinese medicine(TCM)syndrome scores,sex hormones in serum,and hemorrheology indexes were compared between the two groups.The clinical effects,ovulation rates,pregnancy rates,and incidence of adverse reactions were compared between the two groups.Results:The total effective rate,ovulation rate and pregnancy rate in the treatment group were higher than those in the control group(P<0.05).After treatment,the TCM syndrome scores and the levels of whole blood low shear viscosity,whole blood high shear viscosity,and plasma viscosity in the two groups were decreased when compared with those before treatment(P<0.05),and the TCM syndrome scores and hemorrheology indexes in the treatment group were lower than those in the control group(P<0.05).After treatment,the levels of testosterone(T),follicle stimulating hormone(FSH),luteinizing hormone(LH),and dehydroepiandrosterone(DHEAS)in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of sex hormone binding globulin(SHBG)were increased(P<0.05);the levels of T,FSH,LH,and DHEAS in the treatment group were lower than those in the control group(P<0.05),and the SHBG level was higher than that in the control group(P<0.05).There was no significant difference being found in the comparison of the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Xiaozheng Huoxue Yishen Prescription combined with Ethinyl Estradiol Cyproterone Tablets has a definite curative effect in treating infertility due to polycystic ovary syndrome of kidney deficiency and blood stasis type.It can effectively enhance the ovulation rate and pregnancy rate of patients,regulate the level of sexual hormones in patients,improve the abnormal state of hemorheology,and has high safety.
