摘要
目的总结某院内分泌科2型糖尿病合并感染的情况并探讨其相关危险因素。方法收集2019年3月-2022年6月如皋市人民医院内分泌科收治的250例2型糖尿病患者的临床资料,统计合并感染的2型糖尿病患者例数,采用单因素及多因素logistic回归分析探索2型糖尿病合并感染相关危险因素。结果92例2型糖尿病患者合并感染,感染发生率为36.80%。单因素分析结果显示:女性、年龄≥60岁、病程≥10年、合并慢性阻塞性肺疾病(COPD)、合并高脂血症、合并低白蛋白血症、吸烟史、侵入性操作史、酮症酸中毒史、糖化血红蛋白(HbAlc)≥8%、空腹血糖(FPG)≥11 mmol/L的2型糖尿病患者合并感染的风险高于男性、年龄<60岁、病程<10年、未合并COPD、未合并高脂血症、未合并低白蛋白血症、无吸烟史、无侵入性操作史、无酮症酸中毒史、HbAlc<8%、FPG<11 mmol/L的2型糖尿病患者,差异均有统计学意义(χ^(2)=5.252、4.765、6.156、5.116、5.846、8.167、10.068、5.359、4.681、11.358、7.000,P=0.022、0.029、0.013、0.024、0.016、0.004、0.002、0.021、0.030、0.001、0.008)。多因素logistic回归分析结果显示:性别(OR=1.575,95%CI:1.104~1.575,P=0.013)、低白蛋白血症(OR=2.604,95%CI:1.366~4.962,P=0.004)、HbAlc(OR=4.402,95%CI:1.539~12.586,P=0.006)、FPG(OR=3.360,95%CI:1.171~9.645,P=0.025)是2型糖尿病患者合并感染的危险因素。结论2型糖尿病患者合并感染风险较高,女性、合并低白蛋白血症、HbAlc≥8%、FPG≥11 mmol/L均为2型糖尿病合并感染的危险因素。
Objective To summarize the situation of type 2 diabetes complicated with infection in the Department of Endocrinology of the Rugao People's Hospital and explore its related risk factors.Methods The clinical data of 250 patients with type 2 diabetes admitted to the Department of Endocrinology of the Rugao People's Hospital from March 2019 to June 2022 were collected;the number of patients with type 2 diabetes complicated with infection was counted,and the risk factors of type 2 diabetes complicated with infection were explored by single factor and multi factor logistic regression analysis.Results 92 patients with type 2 diabetes were infected,and the infection rate was 36.80%.Univariate analysis results showed:female,age≥60 years old,disease duration≥10 years,chronic obstructive pulmonary disease(COPD),hyperlipidemia,hypoalbuminemia,smoking history,history of invasive procedures,ketosis type 2 diabetes patients with a history of acidosis,glycosylated hemoglobin(HbAlc)≥8%,and fasting plasma glucose(FPG)≥11 mmol/L had a higher risk of infection than men,age<60 years old,disease duration<10 years,and those without COPD,type 2 diabetes patients without hyperlipidemia,without hypoalbuminemia,with no history of smoking,no history of invasive procedures,no history of ketoacidosis,HbAlc<8%,FPG<11 mmol/L;the differences were all statistically significant(χ^(2)=5.252,4.765,6.156,5.116,5.846,8.167,10.068,5.359,4.681,11.358,7.000,P=0.022,0.029,0.013,0.024,0.016,0.004,0.002,0.021,0.030,0.001,0.008).The results of multivariate logistic regression analysis showed:gender(OR=1.575,95%CI:1.104-1.575,P=0.013),hypoalbuminemia(OR=2.604,95%CI:1.366-4.962,P=0.004),HbAlc(OR=4.402,95%CI:1.539-12.586,P=0.006)and FPG(OR=3.360,95%CI:1.171-9.645,P=0.025)were risk factors for infection in patients with type 2 diabetes.Conclusions Type 2 diabetes patients had a higher risk of infection.Women,hypoalbuminemia,HbAlc≥8%,FPG≥11 mmol/L were all risk factors for type 2 diabetes with infection.
