摘要
目的25羟维生素D3[25(OH)D_(3)]在反复呼吸道感染(RRI)患儿中的表达及与TB淋巴细胞亚群的相关性分析。方法选取2020年9月-2022年3月在保定市儿童医院就诊的74例RRI患儿为RRI组,另取同期体检正常的35名儿童为对照组,检测两组受试者25(OH)D_(3)水平、TB淋巴细胞亚群(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、B淋巴细胞)水平,分析不同25(OH)D_(3)水平与TB淋巴细胞亚群的关系。结果RRI组25(OH)D_(3)、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、B淋巴细胞水平分别为(55.41±3.56)nmol/L、(48.35±5.47)%、(26.44±4.23)%、(1.19±0.16)、(9.12±2.54)%,低于对照组的(62.28±5.31)nmol/L、(57.96±6.39)%、(38.57±4.64)%、(1.49±0.22)、(15.49±2.80)%,差异均有统计学意义(t=7.981、8.107、13.548、8.069、11.827,P均<0.001)。25(OH)D_(3)缺乏、25(OH)D_(3)不足及25(OH)D_(3)正常组中,CD3^(+)水平分别为(41.96±4.07)%、(49.71±4.35)%、(58.30±4.63)%,差异有统计学意义(F=63.385,P<0.001);CD4^(+)水平分别为(20.13±3.45)%、(27.57±4.73)%、(34.21±4.26)%,差异有统计学意义(F=47.959,P<0.001);CD4^(+)/CD8^(+)水平分别为(0.71±0.07)、(1.33±0.09)、(1.57±0.12),差异有统计学意义(F=427.415,P<0.001);B淋巴细胞水平分别为(6.08±1.30)%、(8.87±2.15)%、(14.12±2.68)%,差异有统计学意义(F=67.012,P<0.001)。25(OH)D_(3)水平与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、B淋巴细胞水平均成正相关(r=0.683、0.715、0.734、0.561,P均<0.05)。结论25(OH)D_(3)在RRI患儿中的表达水平降低,其水平与TB淋巴细胞亚群密切相关。
Objective To explore the expression of 25 hydroxyvitamin D3[25(OH)D_(3)]and its correlation with TB lymphocyte subsets in children with recurrent respiratory tract infection(RRI).Methods A total of 74 children with RRI treated in the Baoding Children's Hospital were enrolled as RRI group between September 2020 and March 2022,while 35 normal children undergoing physical examination during the same period were enrolled as control group.The levels of 25(OH)D_(3)and TB lymphocyte subsets(CD3^(+),CD4^(+),CD4^(+)/CD8^(+),B lymphocytes)in both groups were detected.The relationship between different 25(OH)D_(3)level and TB lymphocyte subsets was analyzed.Results The levels of 25(OH)D_(3),CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and B lymphocytes in RRI group were(55.41±3.56)nmol/L,(48.35±5.47)%,(26.44±4.23)%,(1.19±0.16)and(9.12±2.54)%,respectively,lower than those in control group[(62.28±5.31)nmol/L,(57.96±6.39)%,(38.57±4.64)%,(1.49±0.22),(15.49±2.80)%],all the differences were statistically significant(t=7.981,8.107,13.548,8.069,11.827;all P<0.001).In 25(OH)D_(3)deficiency group,25(OH)D_(3)insufficiency group and 25(OH)D_(3)normal group,CD3^(+)levels were(41.96±4.07)%,(49.71±4.35)%and(58.30±4.63)%,respectively(F=63.385,P<0.001),CD4^(+)levels were(20.13±3.45)%,(27.57±4.73)%and(34.21±4.26)%,respectively(F=47.959,P<0.001);CD4^(+)/CD8^(+)levels were(0.71±0.07),(1.33±0.09)and(1.57±0.12),respectively(F=427.415,P<0.001),and levels of B lymphocytes were(6.08±1.30)%,(8.87±2.15)%and(14.12±2.68)%,respectively(F=67.012,P<0.001).The level of 25(OH)D_(3)was positively correlated with levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and B lymphocytes(r=0.683,0.715,0.734,0.561;all P<0.05).Conclusion The expression level of 25(OH)D_(3)was decreased in RRI children,which was closely related to TB lymphocyte subsets.
