儿童重症监护病房重症流感病毒感染死亡的影响因素

Influencing factors of death caused by severe influenza virus infection in children’s intensive care unit

目的探讨儿童重症监护病房重症流感病毒感染死亡的影响因素。方法选取2016年6月-2019年6月南阳市中心医院重症监护病房重症流感病毒感染的患儿共113例,其中死亡组32例,存活组81例。收集性别、年龄、空腹血糖、发热持续时间、血白细胞、血淋巴细胞比例、血中性粒细胞比例、乳酸脱氢酶、肌酸激酶同工酶、肌酸激酶、原发病、并发症、呼吸机辅助通气情况等进行分析,观察这些因素与重症流感病毒感染死亡之间的相关性。结果两组患儿在发生重症流感病毒感染后,咳痰、乏力、胸痛、头痛、肺部啰音、甲流H1N1、甲流H3N2对于死亡的影响因素有统计学意义(P均<0.05)。空腹血糖>8.00 mmol/L、血白细胞>11.00×109/L、血淋巴细胞比例≤18.00%、血中性粒细胞比例>76.00%、原发病为非呼吸系统疾病、并发症、呼吸机辅助通气是重症流感病毒感染死亡的危险因素(P均<0.05)。结论影响重症流感病毒感染患儿死亡的因素较多,临床上应该早期防治。

Objective To explore the influencing factors of death caused by severe influenza virus infection in children’s intensive care unit.Methods From June 2016 to June 2019,113 children with severe influenza virus infection in ICU were selected,including 32 in death group and 81 in survival group.Data for sex,age,fasting blood glucose,duration of fever,white blood cells,proportion of blood lymphocytes,proportion of blood neutrophils,lactate dehydrogenase,creatine kinase isoenzymes,creatine kinase,primary disease,complications and ventilator-assisted ventilation were collected and analyzed for the correlation between these factors and the death of severe influenza virus infection.Results After severe influenza virus infection,the clinical manifestations such as expectoration,fatigue,chest pain,headache,lung rale,H1N1 and H3N2are statistically significant for the influencing factors of death prognosis(all P<0.05).Fasting blood glucose>8.00 mmol/L,blood leukocyte>11.00×10~9/L,proportion of lymphocytes≤18.00%,proportion of granuloids in blood>76.00%,primary nonrespiratory diseases,complications and ventilator assisted ventilation were risk factors for death from severe influenza virus infection(all P<0.05).Conclusion There are many factors that affect the death of children with severe influenza virus infection,so early prevention and treatment should be carried out promtly.
