γ1肌动蛋白和心肌肌动蛋白α1表达水平在重症登革热中的临床意义

摘要 目的探究γ1肌动蛋白(ACTG1)和心肌肌动蛋白α1(ACTC1)在重症登革热中的临床意义和预测价值。方法2016-2019年在福建医科大学孟超肝胆医院首次确诊为登革热的住院患者中,收集符合研究要求的共171例不同症状类型的登革热患者血清,分为普通型组(n=78)、重症倾向组(n=69)和重症组(n=24),同时收集61名健康人血清作为健康组,通过酶联免疫吸附法(ELISA)检测登革热患者及健康人血清中ACTG1、ACTC1的表达量,同时绘制不同临床类型登革热患者ACTC1、ACTG1的受试者工作特征(ROC)曲线,进而分析ACTG1、ACTC1对重症登革热的诊断价值。结果健康组、普通型组、重症倾向组、重症组中,血清ACTC1表达值分别为(0.08+0.05)、(0.15+0.08)、(0.20+0.14)、(0.25+0.16)ng/mL,普通型组小于重症倾向组、重症组,差异均有统计学意义(t=1.56、1.98,P均<0.05)。四组中,血清ACTG1表达值分别为(195.67+85.71)、(234.81+68.39)、(260.74+90.63)、(327.59+110.95)ng/mL,重症组和重症倾向组中ACTG1表达量均高于普通组中ACTG1的表达量,差异均有统计学意义(t=1.98、1.97,P均<0.05)。ACTC1和ACTG1预测登革热患者重症倾向的ROC曲线下面积分别为0.71(P<0.05)、0.61(P<0.05)。结论ACTG1与ACTC1随着登革热病情的加重,表达量呈现上升趋势,且ACTG1与ACTC1较血小板在重症登革热患者中具有更好的预测价值,可作为重症登革热的预测指标。 Objective To explore the clinical significance and predictive value ofγ1 actin(ACTG1)and myocardial actinα1(ACTC1)in severe dengue fever.MethodsFrom 2016 to 2019,in the hospitalized patients who were first diagnosed with dengue fever in Mengchao Hepatobiliary Hospital of Fujian Medical University,a total of 171 patients with dengue fever with different symptom types were collected for the study,divided into normal group(n=78),severe tendency group(n=69),and severe group(n=24),and 61 healthy human serums were collected as research objects,and the expression of ACTG1 and ACTC1 levels in dengue patients and healthy human serums were detected by enzyme linked immunosorbent assay(ELISA),and the expression of ACTC1 and ACTG1 levels in dengue patients with different clinical types were analyzed.The diagnostic values of ACTG1 and ACTC1 for severe dengue fever were analyzed by plotting the receiver operating curve(ROC).ResultsThe expression of ACTC1 in healthy group,normal group,severe tendency group and severe group were(0.08+0.05),(0.15+0.08),(0.20+0.14),(0.25+0.16)ng/m L,respectively.The normal group was smaller than the severe tendency group and the severe group,and the differences were statistically significant(t=1.56,1.98,all P<0.05).In the four groups,the expression values of serum ACTG1 were(195.67+85.71),(234.81+68.39),(260.74+90.63),(327.59+110.95)ng/m L,respectively.The expression of ACTG1 in severe group and severe tendency group was higher than that in normal group,and the differences were statistically significant(t=1.98,1.97,all P<0.05).The areas under the ROC curve of ACTC1 and ACTG1 predicted severe tendency of dengue patients were 0.71(P<0.05),0.61(P<0.05).ConclusionsThere was an upward trend in the expression of ACTG1 and ACTC1 levels as dengue fever worsens.It was also found that ACTG1 and ACTC1 had better predictive value in patients with severe dengue fever than the platelets and could be used as a predictive indicator of severe dengue fever.
