影响幽门螺旋杆菌感染消化性溃疡患者病原菌根除效果的因素调查

Factors affecting the eradication of pathogenic bacteria in patients with Helicobacter pylori caused peptic ulcer

目的探讨影响幽门螺旋杆菌(Hp)感染消化性溃疡患者病原菌根除效果的相关因素。方法选取广西壮族自治区脑科医院2018年6月-2019年6月收治的96例Hp感染消化性溃疡患者作为研究对象,患者均行Hp根除治疗,根据治疗效果分为根除成功组和根除失败组;采用χ^(2)检验对患者根除效果的影响因素进行分析,并以非条件logistic回归检验进行多因素分析。结果两组患者年龄、治疗依从性、吸烟习惯史、克拉霉素耐药、阿莫西林耐药等指标比较差异均有统计学意义(χ^(2)=4.714、4.783、4.320、4.921、4.290,P均<0.05);经多因素分析显示,吸烟习惯史、克拉霉素耐药、阿莫西林耐药、治疗依从性等均为影响Hp感染消化性溃疡患者病原菌根除效果的独立危险因素(χ^(2)=2.726、6.787、5.707、5.695,P均<0.05)。结论影响Hp感染消化性溃疡患者病原菌根除效果的因素较多,吸烟习惯史、克拉霉素耐药、阿莫西林耐药、治疗依从性等均为常见影响因素,及时根据相关影响因素制定措施进行干预对提高患者治疗效果有重要价值。

Objective To investigate the factors that influence the eradication of pathogenic bacteria in patients with Helicobacter pylori(Hp)caused peptic ulcer.Methods Ninety-six patients with Hp infection and peptic ulcer who were admitted to the Brain Hospital of Guangxi Zhuang Autonomous Region from June 2018 to June 2019 were selected as the research objects.All patients underwent Hp eradication therapy.The patients were divided into successful eradication group and failure eradication group based on the treatment effect.The facters affecting eradication effect were analyzed by univariate analysis and multivariate analysis.Results Age,treatment compliance,history of smoking habits,clarithromycin resistance,and amoxicillin resistance were significantly different between the two groups(χ^(2)=4.714,4.783,4.320,4.921,4.290,all P<0.05).Multivariate analysis showed,smoking habit history,clarithromycin resistance,amoxicillin resistance and treatment compliance were the independent risk factors that affect the eradication effect of pathogenic bacteria(χ^(2)=2.726,6.787,5.707,5.695,all P<0.05).Conclusions There were many factors that could affect the eradication effect of pathogenic bacteria in patients with Hp caused peptic ulcer.Smoking habits,clarithromycin resistance,amoxicillin resistance and treatment compliance were common influencing factors.Measures should be formulated in time according to relevant influencing factors for the eradication of pathogenic bacteria.
