汕头市首例ST-1655型Y群流脑的病原学及流行病学分析

Etiology and epidemiological analysis on the first ST-1655 complex serogroup Y Neisseria meningitidis isolated from patient in Shantou

目的对2019年8月汕头市发现的首例Y群流脑病例进行病原学及流行病学分析。方法对标本进行革兰染色、生化试验、血清分型、PCR鉴定、多位点序列分型(MLST)和药敏鉴定。结果该病例鉴定结果为脑膜炎奈瑟菌引起,血清分型为Y群,PCR鉴定结果为Y群,MLST基因型为ST-1655型,属CC-23克隆群。药敏鉴定显示对甲氧苄啶/磺胺甲恶唑耐药。结论该病例是汕头市首例Y群流脑病例,是广东省第2例Y群流脑病例,提示要加强流脑菌群和MLST监测。

Objective To analyze the pathogenic character of the first serogroup Y epidemic cerebrospinal meningitis case and close contacts in Shantou in August,2019.MethodsGram stain,biochemical test,serotyping,polymerase chain reaction(PCR)and multilocus sequence typing(MLST)were used to identify the pathogenic bacterial strain in the sample.Meanwhile,drug sensitivity tests were applied in the identification.Results The identification result indicated this case was caused by Neisseria meningitidis.Its serotype and the PCR identification results indicated this strain belonged to group Y,and the MLST genotype was ST-1655,belonging to the CC-23 clone group.As well,this strain showed resistance to sulfonamides(SMZ).Conclusions This is the first case of serogroup Y Neisseria meningitidis in Shantou and the second case in Guangdong province.It was suggested that we should strengthen the surveillance of meningococcal flora and MLST.
