2011-2020年上海市奉贤区晚期血吸虫病医疗救助分析

Analysis of medical assistance for advanced schistosomiasis in Fengxian district,Shanghai from 2011 to 2020

目的本文通过调查2011-2020年上海市奉贤区晚期血吸虫病医疗费用补助和政策知晓情况,旨在为上海市修订晚期血吸虫病医疗费用补助政策和加强针对性宣传、培训提供依据。方法回顾性收集、分析上海市奉贤区2011-2020年晚期血吸虫病病例相关医疗费用与补助资料,探讨相关防控措施。结果2011-2020年新增晚期血吸虫病病例214例,期间累计晚期血吸虫病相关医疗费用补助732次,累计补助金额2201824元;晚期血吸虫病补助金额占自负医疗费用的28.40%;年医疗费用补助总额由2011年的92415元增加到2020年的244345元,增加幅度达264.69%。在册病例年度平均自负金额在8000元(补助上限)以下占58.68%,而新增病例仅为28.22%,新增晚期血吸虫病病例相关医疗费用高于在册病例(χ^(2)=78.554,P<0.05)。93.38%在册晚期血吸虫病病例认为晚期血吸虫病医疗救助政策是项好政策,但政策的知晓率仅为46.00%,知晓的主要途径是通过卫生工作人员和其他受助者;医务人员知晓率仅29.92%。结论新增晚期血吸虫病病例疾病医疗费用负担较重,建议提高晚期血吸虫病相关医疗费用补助标准,同时提高居民和医务人员的政策知晓率。

Objective By investigating the medical expense subsidy and policy awareness of advanced schistosomiasis in Fengxian district,Shanghai from 2011 to 2020,it aimed to provide a basis for Shanghai to revise the advanced schistosomiasis medical expense subsidy policy and strengthen targeted publicity and training.MethodsThe medical expenses and subsidies related to advanced schistosomiasis cases in Fengxian district,Shanghai from 2011 to 2020 were collected and retrospectively analyzed,and relevant prevention and control measures were discussed.ResultsFrom 2011 to 2020,214new cases of advanced schistosomiasis were added.During the period,a total of 732 medical expenses related to advanced schistosomiasis were subsidized,and the cumulative subsidy amount was 2201824 yuan.The subsidy amount for advanced schistosomiasis accounted for 28.40%of the self-paid medical expenses.The total annual medical expense subsidy had increased from 92415 yuan in 2011 to 244345 yuan in 2020,with an increase of 264.69%.The annual average self-pay amount of registered cases was below 8000 yuan(the upper limit of subsidy);registered cases were accounting for58.68%,while the new cases accounting for 28.22%.The medical expenses related to advanced schistosomiasis in new cases were higher than those in registered cases(χ^(2)=78.554,P<0.05).93.38%of the registered advanced schistosomiasis cases thought the advanced schistosomiasis medical assistance policy was a good policy,but the awareness rate of the policy was only 46.00%,and the main way of knowing was through health workers and other recipients;the awareness rate of medical staff was only 29.92%.ConclusionsThe new cases of advanced schistosomiasis had a heavy burden of medical expenses,and it was recommended to increase the subsidy standard for medical expenses related to advanced schistosomiasis.At the same time increase the awareness rate of the policy.
